532nm半导体激光光凝治疗糖尿病视网膜病变

【摘要】 目的观察532半导体纯绿激光视网膜光凝(RP)治疗糖尿病视网膜病变(DR)的临床效果。方法应用532nm半导体纯绿激光对164例324只眼DR患者行RP治疗,观察光凝前后患者视力、眼底及眼底血管造影(FFA)的变化。结果随访3~20个月,在164例324只眼DR患者中,有效285只眼,总有效率87.9%。239只眼增殖前期糖尿病视网膜病变(PPDR)中有效225只眼,有效率94.1%;95只眼增殖期糖尿病视网膜病变(PDR)中有效60只眼,有效率63.2%。PPDR视力提高或不变221只眼,占92.5%;PDR视力提高或不变66只眼,占69.5%。结论RP治疗DR是安全有效的方法。PPDR患者行RP治疗对控制和延缓病情发展,稳定患者视力有重要意义,光凝效果优于PDR。合理选择激光参数可以减少视网膜损伤。

【Abstract】 Objective To evaluate the clinic therapeutic effects of retinal photocoagulation(RP) with 532 nm laser in diabetic retinopathy(DR).Methods Totally 164 cases(324 eyes)of DR underwent RP with Alcon Eyelite Green Laser 532,the changes in visual acuity,fundus and fundus fluorescein angiography(FFA) were analyzed following the RP.Results From 164 cases(324 eyes) of DR 285 eyes were effective during following up from 3 months to 20 months,the total effective rate was 87.9%.And 225 eyes in 239 eyes of preproliferative diabetic retinopathy(PPDR) were effective,the effective rate was 94.1%.Sixty eyes in 90 eyes of proliferative diabetic retinopathy(PDR) were effective,the effective rate was 63.2%.Visual acuity was improved or maintained in 221 eyes(92.5 %) of PPDR and in 66 eyes(69.5 %) of PDR.Conclusions RP is safe and effective in the treatment of DR.It’s very important for the cases of PPDR with RP to control the development of DR and stabilize the vision.The effect of RP in PPDR is better than in PDR.Reasonable laser parameters are benefit to reducing the retinal injury.
