二维应变对室壁运动正常冠心病的应用研究

【摘要】 目的应用二维应变超声心动图(2DSE)定量分析常规方法显示室壁运动正常冠心病患者的左室收缩运动,探讨其临床应用意义。方法采集经冠脉造影证实的38例冠心病和31例对照组患者的心尖长轴、四腔和二腔观的二维灰阶动态图像,应用二维应变软件测量左室壁各节段的收缩应变、应变率及整体长轴应变(GLS),心尖双平面Simpson法测量左室射血分数(LVEF),比较两组参数值及分析LVEF与GLS的相关性。结果组间比较,各项常规超声指标均无统计学差异(P>0.05),部分节段的应变、应变率及GLS有统计学差异(P<0.05或P<0.01),LVEF与GLS呈良好相关性P<0.05)。结论二维应变与应变率互为补充,对评价缺血心肌的敏感性高于常规超声心动图。

【Abstract】 Objective To investigate the clinical value of two-dimensional strain echocardiography (2DSE) in quantifing global and regional myocardial strain of coronary heart disease (CHD) patients with normal wall motion. Methods There were 38 patients with coronary heart disease and 31 healthy age-matched subjects proved by selected coronary angiography.The two dimensional loop-cinec were obtained in apical 4-chamber view, apical 2 chamber view and long axis of left ventricle view.Strain, strain rate and global systolic strain (GLS) were measured using two-dimensional strain software.Left ventricle ejection fraction (LVEF) was calculated by bi-plane Simpson’s method. Values were compared between the 2 groups.The correlations between LVEF and GLS were analyzed respectively. Results Conventional echocardiographic parameters had no statistical difference between the two groups (P>0.05). Strain and strain rate have significant differences at several segments (P<0.05 or P<0.01).GLS has good correla tions with LVEF (P<0.05).Conclusions 2DSE has higher sensitivity than conventional echocardiography in the diagnosis of myocardial ischemia.
