【摘要】 目的探讨平均红细胞体积(MCV)在筛查孕妇地中海贫血(以下简称地贫)中的应用价值。方法以检测作为地中海贫血的初筛试验,对初筛试验阳性的402例孕妇(试验组)进行地贫的基因检测,以了解MCV方法的敏感性;用地贫基因诊断为非地贫者200例(对照组),采用自动血细胞分析仪进行MCV测定,分析MCV筛查方法的特异性。结果(1)用MCV筛查阳性的402例中确诊为地贫的为391例,200例(对照组)非地贫患者用MCV检测为阴性的有159例,敏感性为97.26%,特异性为79.5%。(2)3632例孕妇中确诊地贫的有391例,发病率为10.76%,其中α型地贫297例,占发病总人数的75.95%;β型地贫94例,占发病总人数的24.05%。结论(1)MCV可作为筛查孕妇地贫的有效指标。(2)做好产前地贫的筛查,对预防和降低重型地贫患儿出生具有重要意义。更多还原

【Abstract】 OBJECTIVE To analyze and evaluate the clinical value of mean corpuscular volume ( MCV) from peripheral blood in screening and diagnosing halassemial.METHODS All persons at reproduction-age were examined by MCV and RBC fragile test, and thalassemia gene detection on those persons were suspected. RESULTS (1) All persons were checked on mediterranean anemia by MCV test in prenatal diagnosis. Among them, 402 cases had MCV <80fI were abnormal which could be seen in all gene types of thalassemia.(2)There were 391 cases mediterranean anemia among them. The rate of mediterranean anemia was 10.76%. The rate of α- thlassemia was 75.95% (297cases), and 24.03% (94cases) in β-thalassemia.CONCLUSION (1)MCV test take important clinical value in screening and diagnosing thalassemia. (2)It is necessary to examine blood samples on thalassemia among persons at reproduction-age, and then check thalassemia gene if it is necessary.更多还原
