【摘要】 目的:探讨苯肾上腺素静脉滴注结合晶体液快速扩容与单独苯肾上腺素注入相比较在剖腹产术脊髓麻醉期间预防低血压的功效。方法:选择准备行剖腹产术患者106例,ASAⅠ～Ⅱ级,随机分成两组,完成脊髓麻醉以后立即开始静脉滴注苯肾上腺素100μg/min维持收缩血压接近基础值,直到切开子宫。此外,患者接受乳酸林格氏液输注,A组维持最小输注率,以保持静脉通畅;B组迅速输注。比较两组患者的血液动力学的变化和新生儿的状况。结果:发生低血压患者数:B组1例,A组15例(P<0.05)。两组患者平均收缩血压和心率的最低值及苯肾上腺素总用量及输入率比较的差异有统计学意义(P<0.05);两组之间新生儿观测结果和母亲的副作用比较的差异无统计学意义。结论:苯肾上腺素注入联合快速的晶体液输入可有效预防剖腹产术脊髓麻醉期间低血压的发生。更多还原

【Abstract】 Objective:To explore the combination of phenylephrine infusion and crystalloid rapid expansion alone,compared with phenylephrine injection in the caesarean section to prevent hypotension during spinal anesthesia effect.Methods:106 cases of female patients who would carry out the hysterotokotomy and were ASA Ⅰ～ Ⅱ were randomly divided into two groups.After spinal anesthesia intravenous the patients was immediately intravenous dripped phenylephrine 100 μg /min in order to keep systolic blood pressure until the uterine incision.In addition,Ringer’s solution was intravenous dripped in group A by a minimum infusion rate and tin group B by the rapid infusion.The mothers hemodynamic changes and the newborn status of two groups were compared.Results:15 of group A and 1 of group B occurred hypotension(P<0.05).The differences of average systolic blood pressure,the lowest heart rate,total amount and input rate of phenylephrine were statistically significant between two groups(all P<0.05).The differences of newborn status and maternal side effects between the two groups were not statistically significant.Conclusion:Phenylephrine injection joint rapid infusing crystalloid may be effective in preventing hypotension during spinal anesthesia occurred.更多还原
