脑梗死患者血浆同型半胱氨酸水平的相关因素分析

【摘要】 目的研究脑梗死患者血清同型半胱氨酸(Hcy)水平的变化,观察同型半胱氨酸与脑血管狭窄数量的关系。方法对330例脑梗死患者利用高效液相色谱荧光法,测定血浆Hcy水平,并应用3.0t的MRI仪器检测脑血管狭窄数量,进行比较及相关分析。结果脑梗死患者中多发血管狭窄患者的Hcy水平明显高于单发狭窄及无明显狭窄者,高同型半胱氨酸血症与肌酐水平呈正相关,与性别年龄、血脂水平、凝血时间、纤维蛋白原及尿酸水平无显著相关性,与脑梗死的OCSP分型无关,初发脑梗死与复发脑梗死之间Hcy水平无明显差别。结论脑梗死患者中高同型半胱氨酸血症与脑血管狭窄数量明显相关,可能预示着脑动脉硬化的严重程度。

【Abstract】 Objective To examine the changes of plasma homocysteine in cerebral infarction,to research whether there was the relationship between homocysteine and the number of cerebral vascular stenosis.Methods In 330 patients with cerebral infarction,fasting serum homocysteine was measured with the mothod of high performance liquid chromatography(HPLC).And MR angiography was performed with a GE Signa 3.0-T scanner for the number of vascular stenosis.Results In the patients with cerebral infarction,the mean homocysteine concentrations of patients with multiple stenotic vessels were higher than those of patients with single stenotic vessel and subjects without stenosis.There was close correlation between serum creatinine and homocysteine.However there was no significant correlation between plasma homocysteine levels and age,levels of serum cholesterol,LDL cholesterol,HDL cholesterol,triglyceride,prothrombin time,partial thromboplastin time,fibrinogen,uric acid,OCSP subtype of cerebral infarction,and no difference of homocysteine between first incidence and recurrence cerebral infarction.Conclusion In the patients with cerebral infarction,the hyperhomocystienemia was significantly associated with the number of stenosed vessels and may predict the severity of cerebral atherosclerosis in patients with cerebral infarction.
