【摘要】 目的探讨老年高龄股骨颈骨折患者合并糖尿病行人工全髋关节置换时围手术期的处理方法及经验。方法回顾分析我科自1993年1月～2003年5月间行全髋关节置换治疗高龄股骨颈骨折合并糖尿病患者31例围手术期处理的经验。其中男性13例,女性18例;年龄65～82岁,平均年龄69.7岁。骨折按Garden分型:Ⅰ型2例、Ⅱ型6例、Ⅲ型12例、Ⅳ型11例。有27例患者获得5～60个月的随访,平均随访21个月。结果本组患者经良好的血糖控制均安全度过围手术期。围手术期无死亡患者,1例患者急性感染,经清创冲洗后治愈。术后Harris评分89.0±4.7。结论高龄股骨颈骨折合并糖尿病患者,如无其他手术禁忌证,只要围手术期处理得当,严格控制血糖,均可行人工全髋关节置换,可明显提高生活质量,减少并发症及死亡率。更多还原

【Abstract】 Objective To discuss the treatment experences of total hip arthroplasty for aged patients with femoral neck fracture and diabetes mellitus.Methods The experiences of curing such patients in our department were summarized.From January 1993 to May 2003,31 cases with femoral neck fracture and diabetes mellitus in old persons beyond 65 years old were treated.Thirteen of them were males and eighteen were female,aging from 65 to 92 years old with an average 69.7 years.According to Garden,s classification,there were two cases of Ⅰtype,six cases of Ⅱtype,twelve cases of Ⅲ type,eleven cases of Ⅳ type.Follow-up was conducted on 27 patients for 5 months to 60 months with 21 months on average.Results The blood glucose of all the patients in our group were controlled well.These patients recovered through the peri-operative period.One case infection was found after operated.Non dead case was found.According the Harris,criteria,the score was 89.0±4.7.Conclusion If the blood glucose was controlled in aged patients with femoral neck fracture an diabetes mellitus,they can be cured by total hip arthroplasty,unless there are other contraindiacations.The postoperative complication and mortality would decrease.更多还原
