64排螺旋CT冠脉成像对不同性质粥样斑块的研究

【摘要】 目的探讨64排螺旋CT对不同性质粥样斑块的成像特点与64MSCT评判不同性质斑块所致冠状动脉狭窄程度的可靠性和准确性。方法106例临床疑似和确诊冠心病患者行64MSCT和CAG检查。结果斑块主要分布部位与冠状动脉病变分布规律一致。斑块性质与冠状动脉的狭窄程度无关(r=-0.130,P>0.05)。64MSCT评判所有斑块所致血管狭窄程度与CAG高度一致(K值0.799,P<0.001),评判软斑块、纤维斑块所致狭窄程度的可靠性优于钙化斑块(K值分别为0.839,0.853,0.738,P均<0.001)。64MSCT评判所有斑块所致血管狭窄程度敏感度、特异度、阴性预测值、阳性预测值分别96.7%、88.7%、87.9%、96.9%,对钙化斑块所致病变易过高评价(敏感度、特异度、阴性预测值、阳性预测值分为95.1%、86.7%、97.3%、78%)。结论64MSCT冠状动脉成像能清晰显示冠状动脉内斑块的情况,可以作为一种筛查冠心病无创检查手段应用于临床。

【Abstract】 Objective To explore different characteristics of 64 MSCT atherosclerotic plaque imaging,to evaluate reliability and accuracy of different nature 64MSCT coronary artery stenosis caused by plaque. Methods 106 cases of coronary heart disease patients undergoing coronary angiography 64MSCT and CAG examination. Results The distribution of plaque location and distribution of coronary artery lesions consistent. Plaque nature were unrelated to the degree of coronary artery stenosis(r=0.130,P>0.05)and(K=0.799;K=0.839,0.853,0.738,P<0.001). All 64MSCT judge the degree of vascular stenosis caused by plaque sensitivity, pecificity, negative predictive value,positive predictive value were 96.7%,88.7%,87.9%,96.9% for calcified plaque vulnerable due to overestimate the disease(sensitivity,specificity,negative predictive value,positive predictive value is divided into 95.1%,86.7%,97.3%,78%). Conclusion 64MSCT coronary angiography clearly showed that coronary artery plaque, the screening can be used as a means of noninvasive examination of coronary heart disease by clinical application.
