缺血性脑卒中急性期发生出血性转化的相关危险因素分析
Analysis on Related Risk Factors of Hemorrhagic Transformation in Acute Cerebral Ischemic Stroke
【摘要】 目的探讨缺血性脑卒中急性期发生出血性转化的相关危险因素。方法回顾我院2001年1月~2008年12月96例急性缺血性脑卒中继发HT病例的临床资料,并按照1∶1配对方法,随机抽取同期96例非HT病例做对照组。列入包括梗死面积、栓塞性脑梗死、应用抗凝、溶栓治疗、高胆固醇血症以及尿蛋白阳性等20项可能的因素进行单因素分析,再对可能与HT有关的因素进行条件Logistic回归分析。结果两组资料的Logistic回归分析显示,差异有统计学意义的因素有5项,根据OR值的大小排列为:梗死面积(OR=3.47,95%CI:1.39~8.70,P=0.008)、栓塞性脑梗死(OR=3.43,95%CI:2.42~4.88,P=0.000)、抗凝及溶栓治疗(OR=2.97,95%CI:1.84~4.79,P=0.000)、高胆固醇血症(OR=2.71,95%CI:1.31~5.59,P=0.007)、尿蛋白阳性(OR=2.10,95%CI:1.47~3.00,P=0.000)。结论梗死面积、栓塞性脑梗死、应用抗凝、溶栓治疗、高胆固醇血症以及尿蛋白阳性是与缺血性脑卒中急性期发生HT相关的危险因素。
【Abstract】 Objective To investigate the related risk factors of hemorrhagic transformation(HT) in acute cerebral ischemic stroke.Methods The 96 clinical data of HT in acute cerebral ischemic stroke from January 2001 to December 2008 in our hospital were analyzed retrospectively.With a proportion of 1 to 1,96 patients who were in acute cerebral ischemic stroke but not in HT were taken as control group at random.20 factors were taken in to study including cerebral infarction volume,embolic cerebral infaction,antithrombotic therapy or thrombolytic therapy,hypercholesterolemia,positive proteinuria,etc.Firstly univariate analysis were taken to find the probable risk factors relative to HT,and then multivariate Logistic conditional regression analysis were carried up on these factors.Results The logistic regression analysis showed there were significant differences in 5 factors between the data of 2 groups.According to the orders of OR values,those included cerebral infarction volume(OR = 3.47,95%CI:1.39~8.70,P = 0.008),embolic cerebral infaction(OR = 3.43,95%CI:2.42~4.88,P = 0.000),antithrombotic therapy or thrombolytic therapy(OR = 2.97,95%CI:1.84~4.79,P = 0.000),hypercholesterolemia(OR = 2.71,95%CI:1.31~5.59,P = 0.007),positive proteinuria(OR = 2.10,95%CI:1.47~3.00,P = 0.000).Conclusion The massive cerebral infarction,embolic cerebral infaction,antithrombotic therapy or thrombolytic therapy,hypercholesterolemia and positive proteinuria are the major risk factors of HT in acute cerebral ischemic stroke.
