日光相关性皮肤肿瘤中朗格汉斯细胞数量和形态的观察

【摘要】 目的探讨朗格汉斯细胞(LC)数量及形态变化与日光相关性皮肤肿瘤发病的关系,以及其与良恶性的相关性。方法免疫组化SP法检测正常皮肤、脂溢性角化病(SK)、角化棘皮瘤(KA)、光线性角化病(AK)、基底细胞癌(BCC)及鳞状细胞癌(SCC)各15份标本中CD1a的表达。光镜下观察各标本中LC形态的变化,病理图像分析仪测量各标本单位面积的平均光密度值。结果与正常皮肤比较,SK皮损中LC的数量略有减少(P>0.05),而KA、AK、BCC和SCC皮损中LC的数量显著减少(P<0.05);SK、KA分别与AK、BCC和SCC比较,差异均有显著性意义(P均<0.05)。皮损中LC的形态也发生明显变化,且在皮肤恶性肿瘤中变化更为明显。结论LC数量的减少和功能的降低,促进了皮肤肿瘤的发生,肿瘤的恶性程度越高,LC的数量越少,形态变化越明显。紫外线辐射可能在这些皮肤肿瘤LC的数量和形态变化中起重要作用。

【Abstract】 Objective To investigate the relationship between the change of number and morphous of Langerhans cells and degree of malignancy of solar-associated skin tumors.Methods Respectively,15 paraffin blocks of seborrheic keratosis(SK),keratoacanthoma(KA),actinic keratosis(AK),basal cell carcinoma(BCC) and squamous cell carcinoma(SCC) were collected,15 cases of normal adult skin specimens from sun exposure site were used as control.The expression of CD1a were detected by immunohistochemistry and morphous of Langerhans cells were observed through light microscope.The average optical of unit area of each specimen was analysised by patho-image analysator.Results There was no significant reduction in LC numbers of SK(P>0.05),while a statistically significant reduction in LC numbers of KA,AK,BCC and SCC compared with that in normal skin(P<0.05);and SK and KA were compared to AK,BCC and SCC respectively,there were significant difference(all P<0.05).Except for numbers,the morphous of LC in these skin tumors also changed significantly,especially in malignant tumor of skin.Conclusions Reduction in numbers and function of LC maybe promote the development of skin tumor and showed correlation with the degree of malignancy of the skin tumors,which was in higher degree tumor,the numbers of LC was more fewer and the change of LC,s morphous was more significant.Ultraviolet radiation may play an important role on the change of the number and morphous of LC in these skin tumors
