【摘要】 目的:回顾性调查我院484例铜绿假单胞菌(PA)感染患者的抗菌药物应用情况,为临床合理用药提供依据。方法:对2005年1月～2008年12月住院患者的各种临床标本中分离确认的687株铜绿假单胞菌感染患者抗菌药物使用情况进行统计,分析评价用药合理性。采用药物利用评价的方法计算用药频度,了解临床医师的用药倾向。结果:PA易感人群以老年、伴发多种基础疾病、住院时间长的患者居多,临床感染以肺部感染和尿路感染为主。各类抗菌药物应用比例以第3代头孢菌素类及β-内酰胺酶抑制剂最多、其次是碳青霉烯类和氟喹诺酮类。连续4年用药频度DDDs排列前两位的是哌拉西林/他唑巴坦,头孢哌酮钠/舒巴坦。结论:我院临床医师治疗PA感染以选择β-内酰胺酶抑制剂为主,重视病原学监测指导用药可加强抗菌药物的合理使用。更多还原

【Abstract】 OBJECTIVE: To review the utilization of antibiotics in 484 patients infected with pseudomonas aeruginosa for reference of clinical rational medication.METHODS: The utilization of patients infected with pseudomonas aeruginosa(a total of 687 strains separated from different specimens) from January 2005 to December 2008 in our hospital was analyzed statistically to evaluate the rationality of drug use.The DDDs was analyzed with the use of drug utilization review method,meanwhile,the medication frequency was analyzed.RESULTS: Pseudomonas aeruginosa infection mostly occurred in older patients,those associated with variety of underlying diseases and the patients with long hospital stay.Pseudomonas aeruginosa infection mainly occurred in lung and urinary tract.Pseudomonas aeruginosa infection was treated mainly by the third generation cephalosporins and Beta-lactamase inhibitors followed by carbapenem and fluoroquinolones,with Piperacillin/tazobactam and cefoperazone/sulbactam topping the list over four consecutive years.CONCLUSION: In our hospital,beta-lactamase inhibitors have been used predominantly in the treatment of Pseudomonas aeruginosa infections.Rational use of antibiotics can be strengthened by attaching great importance to the etiologic monitoring and guidance.更多还原
