我院3种清洁手术围手术期抗菌药物应用分析

【摘要】 目的:评价我院甲状腺手术、乳腺手术、疝气修补术(简称"3种清洁手术")围手术期应用抗菌药物的合理性。方法:抽取我院2008年7月~2009年3月3种清洁手术患者出院病历共434例,对抗菌药物种类、用法用量、用药时间等进行分析统计。结果:434例患者中,抗菌药物预防性应用率达100%;采用术前0.5~2 h给药者419例次,占96.32%,术后用药,t<24 h者,123例次,占28.28%,24 h<t≤48 h者,168例次,占38.62%。预防性应用抗菌药物涉及6类,种类最多的为青霉素类(占46.20%),其次为头孢菌素类(占36.33%,但其中第1代头孢菌素类仅占3.68%)。结论:我院3种清洁手术预防性应用抗菌药物存在不合理现象,应引起重视,有效促进围手术期抗菌药物的合理应用。

【Abstract】 OBJECTIVE: To evaluate the rationality of the perioperative utilization of antibiotics in patients undergoing thyroidectomy,adenomammectomy or herniorrhaphy(3 aseptic operations in brief) in our hospital.METHODS: A total of 434 medical records of discharged patients who underwent thyroidectomy,adenomammectomy or herniorrhaphy in our hospital between July 2008 and Mar.2009 were sampled for statistical analysis with regard to the categories of antibiotics,dosage and administration,medication time etc.RESULTS: Of the total 434 cases reviewed,100% received antibiotics prophylactically;96.32%(419) received antibiotics at 0.5~2 before operation,28.28%(123) received antibiotics within 24 h after operation,and 38.62%(168) received antibiotics between 24 h and 48 h after operation.A total of 6 categories of antibiotics were used prophylactically,topping the list were penicillins(46.20%),followed by cephalosporins(36.33%) in which the first generation cephalosporins accounted for 3.68%.CONCLUSION: Great importance should be attached to the irrational prophylactic use of antibiotics in 3 different aseptic operations in our hospital so as to effectively promote rational use of antibiotics.
