【摘要】 目的探讨结肠癌的螺旋CT表现及临床病理特点。方法回顾性分析53例经手术病理证实的结肠癌患者的螺旋CT表现,并与术后病理特点对照,分析螺旋CT诊断与Dukes分期的准确性。结果①53例CT显示肠壁增厚、43例表现为肿块或结节,浆膜面模糊49例,39例病理证实为浆膜面受侵;②24例分化较好的肿瘤呈轻-中度均匀强化,29例分化差的肿瘤呈不均匀强化或分层状强;③CT发现淋巴结转移16例,主要CT表现为不均匀强化或中心低密度周围高密度,边缘不清晰,低密度区CT值小于60 Hu。CT漏诊4例;④对照术后病理Dukes分期,46例分期正确,7例分期错误。结论螺旋CT扫描可有效显示结肠癌的部位、大小和形态,确定肿瘤侵犯范围、有无淋巴结及远处转移等,可以准确的进行术前分期,具有较高的临床应用价值。更多还原

【Abstract】 Objective To evaluate the spiral CT manifestations and clinical pathological features of colon cancer.Methods The spiral CT findings of 53 patients with colonic cancer confirmed by surgery and pathology were analyzed retrospectively and their pathological features were compared.The spiral CT diagnosis and the Dukes staging accuracy were analyzed.Results ①The spiral CT findings showed wall thickening of colon in all 53 cases,mass or nodules in 43 cases,and serous surface fuzzy in 49 cases,39 cases were confirmed for serosal surface infection by pathology.②The tumors with well differentiation appeared as slight-medial homongeneous enhancement in 24 cases,while poorly differentiation tumors appeared as nonhomogenous enhancement or layer-like enhancement in 29 cases.③The spiral CT feature showed metastatic lymph nodes in 16 cases,whose main CT findings were enhancement or low-density center with high-density surroundings,the edge was not clear.The CT value of low-density areas were less than 60Hu.There were 4 cases missed ④Compared with pathologic Dukes staging after operation,46 cases were correct,7 cases were wrong,staging by CT overall accuracy was 86.8%.Conclusions The spiral CT scan can effectively show the location of colon cancer,size and shape,to determine the scope of tumor invasion,lymph node and distant metastasis,and can carry out accurate preoperative staging of high clinical value.更多还原
