自然周期IVF-ET248例临床分析

【摘要】 目的:探讨自然周期体外受精-胚胎移植(NC-IVF-ET)技术在不孕症治疗中的临床应用价值。方法:采用SPSS统计软件包对248例行NC-IVF-ET和2185例行控制性超排卵(短方案COH)助孕的患者促排卵结果进行分析。结果:NC-IVF-ET组的获卵率和临床妊娠率显著低于COH组(P<0.05),周期取消率显著高于COH组(P<0.05),但两组正常受精率、卵裂率和种植率均无显著性差异(P>0.05)。而NC-IVF-ET组的OHSS发生率和多胎妊娠率则显著低于COH组(P<0.05)。在各种原因引起的不孕症中,输卵管因素引起的不孕症临床妊娠率最高,而无获卵是最主要的取消原因。结论:NC-IVF-ET不使用促排卵药物,经济、安全,应在有适应证的不孕症患者中加以应用。

【Abstract】 Objective: To investigate the clinical value of natural cycle in vitro fertilization and embryo transfer (NC-IVF-ET) in the treatment of infertility. Methods: SPSS statistic software was used to analyze the clinical results of 248 patients received the treatment of the protocol of NC-IVF-ET and 2 185 patients received the treatment of controlled ovarian hyper stimulation (short protocol COH). Results: The rate of oocytes retrieved and the rate of clinical pregnancy of NC-IVF-ET group were significantly lower than that of COH group (P<0.05), and the cycle cancellation rate was significantly higher than that of COH group (P<0.05), but the normal rate of fertilization, merogenesis rate and planting rate showed no difference between the two groups (P>0.05). The rate of OSS and the multifetal pregnancy in NC-IVF-ET group were significantly lower than that of COH group (P<0.05). The rate of clinical pregnancy of the infertile couples which caused by ovarian duct obstruction was the highest, and no retrieved oocyte was the main reason of cycle cancelling. Conclusion: The protocol of NC-IVF-ET has the advantages of no need of ovarian stimulation medicine and is more economic and safe, it should be used in some patients who have indications.
