【摘要】 目的:比较单动头置换与双动头置换在高龄老年人(≥85岁)股骨颈骨折中的治疗效果。方法:我院2004～2007年共收治50例高龄老年(≥85岁)股骨颈骨折患者,分为单动头置换组(27例)和双动头置换组(23例)。随访6个月～4年,平均3.4年。对两组患者手术时间、手术出血量、住院时间、住院费用、并发症、关节功能及死亡率等进行比较。结果:两组患者平均手术时间、术中出血量、住院时间、并发症、关节功能Harris评分及死亡率比较,差异无显著性(P>0.05)。住院费用比较,双动头置换组术中多于单动头置换组,差异有显著性(P<0.05)。结论:在治疗高龄老年(≥85岁)股骨颈骨折患者的疗效比较中,双动头置换对比单动头置换没有任何优势,而且住院费用高。对高龄老年(≥85岁)股骨颈骨折患者的治疗尽可能采用简单有效的手术方法,单动头置换完全可以满足该类患者的需要,是一个合理的选择。更多还原

【Abstract】 Objective: To compare with the effects of unipolar and bipolar hemiarthroplasty in treatment of elderly patients (≥85 years) with femoral neck fracture. Methods: Fifty elderly patients with femoral neck fracture were divided into unipolar hemiarthroplasty group (27 cases) and bipolar hemiarthroplasty group (23 cases) from 2004 to 2007 in our hospital. The follow-up period ranged from 6 months to 4 years, with an average of 3.4 years. The operation time, blood lost during operation, hospitalization time, hospitalization expense, complications, joint function and mortality rate of two groups were compared. Results: There was no significant difference between two groups in operation time, blood lost during operation, hospitalization time, complications, Harris scoring of joint function, and mortality rate (P>0.05). The hospitalization expense of bipolar hemiarthroplasty group was more than that of unipolar hemiarthroplasty group (P<0.05). Conclusion: In comparison of the therapeutic effect in treating elderly patients (≥85 years) with femoral neck fracture, bipolar hemiarthroplasty shows no advantage than unipolar hemiarthroplasty, and the hospitalization expense of the former is more than the latter. A simple and ef fective surgery should be selected to treat elderly patients (≥85 years) with femoral neck fracture, and unipolar hemiarthroplasty could completely satisfy the requirement of this kind of patients, and it is a reasonable choice.更多还原
