吸烟与β2 AR-16多态性对哮喘的联合影响
Combination Effect of Gene Polymorphisms in 16 Position of β2-adenergic Receptor and Cigarette Smoking on Asthma in Chinese Han Individuals
【摘要】 目的探讨吸烟与β2肾上腺素受体(β2AR)16位点多态性交互作用与哮喘易感性的关系,以及其联合作用对第1秒用力呼气容积(FEV1)的影响。方法采用成组的病例-对照研究方法,在120例哮喘患者及年龄、性别与之相匹配的120例正常人中用聚合酶链反应产物直接测序确定个体β2AR-16位点基因型,并调查吸烟情况、测定FEV1,进一步分析吸烟与各种基因型哮喘患者的关系以及吸烟与各种基因型相互作用对FEV1的影响。用logistic回归计算比数比值(OR)与95%的可信区间(CI),并调整潜在的混杂因素的影响。结果哮喘组吸烟人数比对照组显著升高(P<0.05)。携有Arg/Arg基因型的人群,吸烟者较不吸烟者相比其哮喘发生的风险显著增加(OR=9.74,95%CI:2.07~43.50),并且随着吸烟剂量的增加哮喘发生的风险也增加,存在明显的剂量反应关系;在哮喘组不吸烟人群中,三种基因型的FEV1不同,组间差异显著(P均<0.05),并且各基因型组内,吸烟人群与不吸烟人群FEV1比较,差异显著(P均<0.05)。结论吸烟与纯合子Arg-16的交互作用增加了易感人群发展为哮喘的危险性,吸烟与β2AR-16位点多态性对哮喘患者FEV1具有协同损伤作用。
【Abstract】 Objective To investigate the association of asthma with gene polymorphisms in 16 position of β2-adenergic receptor(β2-AR-16)and cigarette smoking,and its combination effect on FEV1 in asthma.Methods This was a study of the molecular epidemiology in patients with asthma.The research design was a case-control study.β2-AR-16 gene polymorphisms were detected by sequencing the specific PCR-products directly,FEV1% was measured with respiromete and smoking status was recorded in 120 asthma patients and 120 age and sex matched healthy controls.Further more the association analysis including cigarette smoking with β2-AR-16 gene polymorphisms,and FEV1 with cigarette smoking and β2-AR-16 gene polymorphisms were performed.Multiple logistic regression was used to calculate the odds ratio(OR)and the 95% confidence interval(CI)and to adjust for potential confounding factors.Results The ratio of cigarette smoking in asthma groups was higher than that in normal groups(P=0.0186).Compared with never-smoking Arg-16 homozygotes,those ever-smokers who are Arg-16 homozygotes hold a significantly increased risk of asthma[odds ratio(OR)=9.74;95% confidence interval(CI):2.07 to 43.50].This association showed a clear dose-response relationship with the number of cigarettes smoked.In never-smoking asthmas,subjects with different β2-AR-16 gene polymorphisms had different FEV1,the difference of FEV1 between each of them were significant(all P<0.05),at the same times,in asthmas with different β2-AR-16 gene polymorphisms,the difference of FEV1 between smoking groups and never-smoking groups were also significant(all P<0.05).Conclusion The interaction between cigarette smoking and Arg-16 homozygotes increased the susceptibility of an individual to asthma;Cigarette smoking in coordination with β2-AR-16 gene polymorphisms had impairment effect on FEV1.
