【摘要】 目的与Rutkow术治疗成人原发腹股沟疝比较,探讨Modify Kugel术的优点。方法回顾性分析山东省荣成市中医院2003年2月以来开展的38例(41侧)Rutkow疝环填充式无张力疝修补术及复旦大学附属华山医院腹壁疝微创治疗中心2005年6月至2006年8月以来开展的Modify Kugel无张力疝修补术36例(40侧)的临床资料。结果全组病例按手术情况、术后并发症等,对两种手术方式进行比较,Modify Kugel术术后止痛剂应用次数、平均恢复自主活动时间、切口疼痛及异物感例数均明显减少,P<0.01,有显著差异;平均切口长度、平均手术时间、发热例数、平均住院时间均减少,P<0.05,有统计学意义。结论与Rutkow术比较,ModifyKugel术更符合工程力学原理,适应症更广,免缝合,减少了手术时间及并发症,术后疼痛轻、恢复快;具有微创手术的特点,与腹腔镜手术具有相似的效果。Modify Kugel术是一种具有良好发展潜力的无张力疝修补术。更多还原

【Abstract】 Objective To compare Modify Kugel herniorrhaphy with Rutkow herniorrhaphy in therapeutic efficacy of primarily adult groin. Methods Clinical and followed- up data of 36 cases (40sides) undergoing Modify Kugel herniorrhaphy since June 2005 and 38 cases (41 sides) undergoing Rutkowherniorrhaphy between April 2004 and June 2006 were retrospectively analyzed.All cases were treated in RongCheng Traditional Hospital of ShanDong Province and HuaShan Hospital of ShangHai. Results Compared with Rutkow herniorrhaphy,in the Modify Kugel herniorrhaphy group, postoperative antidyne using、average activity recovery time、pain and foreign body sensation were obviously reduced (P<0.01). Average incision length、average surgical time、cases of fever and average stay were reduced (P<0.05). Conclusion Compared with Rutkow herniorrhaphy,the Modify Kugel herniorrhaphy accords with engineering mechanics principle;Modify Kugel herniorrhaphy has wide indications; Modify Kugel herniorrhaphy reduces operation time、complications、pains and recovery time;Modify Kugel herniorrhaphy are minimally invasive repair 、sutureless and more like laparoscopic hernia repair.As a kind of tension- free hernioplasty,Modify Kugel herniorrhaphy will be used widely.更多还原
