腰椎MRI间盘信号及高度的改变与椎间盘源性腰痛的相关性研究

【摘要】 目的探讨腰椎MRI间盘信号及高度的改变与椎间盘源性腰痛的相关性,为临床应用腰椎MRI信号及高度评价椎间盘源性腰痛提供依据。方法164例椎间盘源性腰痛患者行腰椎MRI平扫并VAS评分,测量腰椎MRIT2加权像上椎间盘的信号强度及高度。根据VAS评分结果分为轻度疼痛组(A组)、中度疼痛组(B组)、重度疼痛组(C组)。检查结果与正常组(80例)进行比较。结果腰椎间盘信号与年龄间的直线回归的相关系数R值接近-1,信号与年龄呈负相关;椎间盘高度与年龄的相关系数R值接近0,椎间盘高度与年龄无关。三组椎间盘信号与正常组相比较及其三组间信号比较,P值都<0.05,随着疼痛症状加重,间盘信号逐渐减弱,;中度疼痛组、重度疼痛组椎间盘高度改变与正常组相比较,P值均<0.05,而轻度疼痛组与正常组差异不明显。椎间盘信号/高度的比值的回归相关系数R值与临近节段关系最为密切,说明椎间盘信号/高度的改变与临近节段的退变程度相关。结论椎间盘MRI的信号及高度改变是椎间盘退变的影像学表现,椎间盘源性腰痛早期椎间盘信号出现减弱而并无椎间隙高度改变,当腰痛程度进一步加重时信号明显减弱并出现椎间高度塌陷,而且椎间盘信号的减弱及高度的丢失可加速相邻节段的退变,为临床诊治椎间盘源性腰痛提供依据。

【Abstract】 Objective To investigate the correlation study of the signal and height of the MR imaging for the lumbar disc and the discogenic low back pain can provide evidence for the evaluation of the discogenic low back pain.Methods MR imaging was performed in 164 patients with discogenic low back pain who administered VAS(visual analogue scale).The signal and height of the disc in T2WI was measured.164 patients with discogenic low back pain divided into three groups depending on the VAS.Results were analyzed by comparing with 80 patients in the normal control group.Results The correlation coefficient R value between age and signal of the lumbar disc was close to-1,which meaned that signal was negatively correlated with age;The correlation coefficient R value between age and height of the lumbar disc was close to 0,which meaned that age was independent of height.Compared to the disc signal among four groups,P <0.05,with the pain gradully aggravate the disc signal became weakened;The moderate pain group and the severe pain group Compared with the nomal group,P <0.05,but the mild pain group had no differenc.The correlation coefficient R-value of the disc signal /height was reduce with the segment increased,the change of the disc signal /height was closely related to the degree of adjacent degeneration.Conclusion The signal and height of the MR imaging for the lumbar disc was a high degree of disc degeneration of the imaging findings.In the early the disc signal became weakened but the height had no change.When the pain aggravated the disc signal became more weakened and the heighth had changed.the change of the disc signal /height was closely related to the degree of adjacent degeneration.It can provide evidence for the evaluation of the discogenic low back pain.
