【摘要】 目的通过与CT对比研究,探讨超声诊断鼻咽癌的应用价值。方法对105例临床症状可疑鼻咽癌患者行超声检查和CT检查后行鼻内窥镜检,发现可疑病灶行鼻内窥镜活检送病理检查,以评价超声检查诊断鼻咽癌的价值。结果超声提示鼻咽癌73例中,68例得到病理证实,5例误诊;两种影像学漏诊2例,经活检送病理确诊。68例鼻咽癌的声像表现为鼻咽部软组织增厚型和肿块型,咽旁间隙受侵63例,骨质破坏3例,颈部淋巴结肿大66例。超声对鼻咽癌的敏感性、特异性、阳性预测值、阴性预测值、准确性分别为97.14%、85.71%、93.15%、93.75%、93.33%,CT对应值分别为97.14%、94.28%、97.14%、94.28%、96.19%。结论超声和CT对鼻咽癌均具有良好的诊断价值,判断临床分期二者有一定的互补性,对于鼻咽局部的受侵改变CT观察优于超声。更多还原

【Abstract】 Objective To study the clinical value of ultrasoound in diagnosis of nasopharyngeal carcinoma,and compared with CT diagnosis.Methods 105 patients with suspicious nasopharyngeal carcinoma received ultrasound and CT examination,then underwent nasendoscopy.When the focus or any dubiety was found,biopsy was sent for pathological examination so as to evaluate the diagnostic value of ultrasound in examining nasopharyngeal carcinoma.Results Of 73 cases indicated with nasopharyngeal carcinoma by ultrasound,68 cases had been proved by pathological examination,5 cases were misdiagnosed and 2 cases missed diagnosis by CT and ultrasound,which were proved by pathological examination.In the 68 cases with nasopharyngeal carcinoma,the sound image presented that nasopharyngeal soft tissue was thick and mass.Lateral pharyngeal spaces in 63 cases were invaded,3 cases had bony destruction,and 66 cases encountered combined enlarged cervical lymph nodes.The sensitivity,specificity,positive prediction value,negative prediction value and accuracy of ultrasound in examining nasopharyngeal carcinoma was 97.14%,85.71%,93.15%,93.75%,93.33%,respectively, while the corresponding values of CT examination was 97.14%,94.28%,97.14%,94.28% and 96.19%,respectively.Conclusion Both ultrasound and CT have good diagnostic values in examining nasopharyngeal carcinoma.Both are complementary each other for estimating clinical stages.As to observing partial invaded nasopharyngeal tissues,CT screening is superior to ultrasound.更多还原
