【摘要】 目的探讨三维适形放疗(3DCRT)和EP方案同步化疗治疗Ⅲ期非小细胞肺癌(NSCLC)的疗效和毒副反应。方法100例患者随机分为治疗组和对照组,每组50例。治疗组采用3DCRT+EP方案,放疗每次3 Gy,总剂量DT66 Gy,22次,4.5周;对照组用常规放疗+EP方案,放疗DT66 Gy,33次,6.5周。两组均于放疗第1、29天各用EP方案化疗一次。结果治疗组有效率84%,明显高于对照组的66%(P<0.05)。治疗组1、3、5年生存率分别为78%、36%、18%,亦明显高于对照组的62%、28%、8%(P<0.05)。治疗组放射性肺炎发生率较对照组低(P<0.05)。结论3DCRT加EP方案同步治疗Ⅲ期非小细胞肺癌,近期疗效较好,提高了生存率,照射野的缩小,使放疗毒副反应减轻,改善了生存质量。更多还原

【Abstract】 Objective To evaluate the efficacy and side effects of concurrent three dimensional conformal radiotherapy(3DCRT) and chemotherapy with EP for stage Ⅲ non-small cell lung cancer(NSCLC).Methods One hundred cases with stage Ⅲ NSCLC were divided into treatment group(A) and control group(B) with 50 cases each.3DCRT combined with EP was adopted in group A,in which radiation dose was Dt66Gy/22F/4.5W.Routine radiation therapy plus EP was used in group B,in which radiation dose was Dt66Gy/33F/6.5W.Both groups were radiated by EP method on the 1st and 29th day.Results The effective rate of group A was 84%,which was higher than 66% of group B(P<0.05).The 1-,3-and 5-year survival rates of group A were 78%,36% and 18%,which were higher than 62%,28% and 8% of group B,respectively(P<0.05).Incidence of radiation pneumonitis in group A was lower than that in group B(P<0.05).Conclusion The regime of 3DCRT and EP for treatment of stage Ⅲ NSCLC has the advantages of good short-term efficacy,improved survival rate,reduced radiation fields,decreased radiation side effects and improved life quality.更多还原
