【摘要】 目的观察早期结膜下注射5-氟尿嘧啶(5-FU)对控制急性闭角型青光眼行小梁切除术患者前房炎性反应的作用。方法将36例(48只眼)青光眼患者施行小梁切除术联合白内障囊外摘除(ECCE)+人工晶体植入(IOL)术,随机分成A、B两组:A组(24只眼),术毕球旁注射地塞米松2.5 mg,同时立即球结膜下注射5-FU 5 mg,术后第1天应用0.1%地塞米松滴眼液及抗生素滴眼液点眼,视前房内纤维素渗出情况隔日注射5-FU 5 mg,总量不超过30 mg。B组(24只眼),术毕球旁注射地塞米松2.5 mg,术后第1天应用0.1%地塞米松滴眼液及抗生素滴眼液点眼;观察术后1、3、7 d及1个月的眼压、滤过泡、前房深度、纤维渗出完全吸收时间和最佳矫正视力恢复时间。结果两组患者术前、术后7 d及术后1个月的眼压差异无统计学意义。(2)术后前房纤维渗出吸收时间,A组和B组的平均时间分别为(5.2±3.4)d vs.(14.2±3.4)d,两组比较,有统计学意义(P<0.01)。恢复至最佳矫正视力的平均时间分别为(8.3±2.4)d vs.(14.3±4.2)d,两组比较,有显著统计学意义。结论术后早期应用5-FU能有效的减轻前房内纤维素样渗出,同时促进患者视力恢复。更多还原

【Abstract】 Objective To observe the effect of early subconjunctival injection of 5-FU in anterior chamber on anti-inflammation in patients underwent acute angle-closure glaucoma surgery.Methods Thirty-six patients(48 eyes) with acute angle-closure glaucoma undergoing combined ECCE-trabeculectomy were divided into two groups.The patients in group A were given subconjunctival injection of 5-FU 5 mg and dexamethasone 2.5 mg at the end of operation,eye drops of 0.1% dexamethasone and antibiotics on the 1st postoperative day,and injection of 5-FU 5 mg every other day according to anterior chamber fiber exudation with the total dose less than 30 mg.The patients in group B(24 eyes) were treated as those in group A except the use of 5-FU.The intraocular pressure(IOP),situation of filtering bleb,anterior chamber depth,the total absorption time of fiber exudation and the best corrected visual acuity were recorded on the 1st,3rd,7th and 30th postoperative day.Results There was no significant differece in IOP between the two groups before opration on the 7th and 30th postoperative day.(2) The average time of fiber exudation absorption significantly shorter in group A than that in group B [(5.2±3.4) days vs.(14.2±3.4) days].So did the time of achieving the best corrected visual acuity [(8.3±2.4) days vs.(14.3±4.2) days].Conclusion Early subconjunctival injection of 5-FU can effectively attenuate the anterior chamber inflammation and promote visual recovery in the patients underwent acute angle-closure glaucoma surgery.更多还原
