【摘要】 目的:探讨胃肠道内、外的胃肠间质瘤(GISTs)的临床表现、影像学及病理学特征。方法:对183例确诊的GISTs(包括167例胃肠道内GISTs和16例胃肠道外GISTs)的临床特点、影像学表现及病理特征进行回顾性分析并检测免疫组化指标(Vimentin、CD117、CD34、Actin、S-100)。结果:胃肠道外GISTs约占8.7%。胃肠道内GISTs和胃肠道外GISTs的临床表现、镜下形态和免疫表型均较相似,主要是发生部位不同,胃肠道外GISTs肿瘤体积较大且生物学行为分组级别较高。结论:胃肠道外GISTs较胃肠道内GISTs除发生部位不同外,还表现为瘤体体积较大、常见上皮细胞样型、出血坏死较突出且生物学行为分组侵袭性及复发率相对较高,影像学CT检查无气液平面。更多还原

【Abstract】 Objective:To study the clinical feature,morphology,immunohistochemical characteristics and imaging features of gas-trointestinal stromal tumors(GIST) and extra-gastrointestinal stromal tumors(EGIST) and to find the difference between GIST and EGIST.Methods:The clinical and pathologic data of 183 patients with GISTs including 167 GIST and 16 EGIST were analyzed retrospectively.Vimentin,CD117,CD34,Actin and S-100 were detected with a panel of immunohistochemistry.Results:The rate of EGIST in GISTs was 8.7%.There were no specific clinical symptoms or signs in GIST and EGIST patients.Therer were no significant difference between GIST and EGIST on the morphology and immunohistochemistry.The most important difference was the originated location of GIST and EGIST.And EGIST was larger and more invasive than GIST.Conclusion:The primary sites of GIST and EGIST are different.EGIST is often composed of epithelial cells.EGIST is bigger and easy to necrotize and hemorrhage grossly.EGIST is also more invasive and reoc-curs easily.And we can not find the air-fluid interface in EGIST.更多还原
