Nd:YAG激光治疗后发性白内障的疗效观察

【摘要】 目的探讨Nd:YAG激光以不同术前处理方式治疗后发性白内障的效果。方法用Nd:YAG激光对58例(58眼)后发性白内障患者分2组进行治疗,A组为散瞳后在接触镜下行激光治疗,B组为不散瞳直接行激光治疗,所有患者术后随访6~12个月。结果58例58眼患者Nd:YAG激光治疗后视力有不同程度提高,增视率100%,A组、B组术后视力与术前比较差异均有显著统计学意义(P均<0.01);B组激光治疗时间(3.5741±0.9168)min较A组(7.4815±1.7180)min缩短,2组比较差异有统计学意义(P<0.05)。2组术后未见严重并发症发生。人工晶状体出现激光损伤斑4眼,玻璃体前界膜破裂14眼,均不影响视力。结论采用不散瞳直接行Nd:YAG激光后囊膜切开术治疗后发性白内障,操作简易且疗效确切。

【Abstract】 Objective To study the effects of Nd:YAG laser with different preoperative managements for after-cataract.Methods Fifty-eight cases(58 eyes) with after-cataract were divided into two groups,and treated with Nd:YAG laser.The pupils were dilated before taking laser treatment under preset lens in group A,and laser treatment was directly taken without dilating pupils in group B.The follow up time was 6 to 12 months.Results Visual acuity improved at different levels after Nd:YAG laser treatment in 58 eyes of 58 cases.The improved rate of visual acuity was 100%.There were significant differences of visual acuity for group A and group B between pre-and post-operation(both P<0.01).The time of laser treatment in group B(3.574 1±0.916 8)minutes was shorter than that of group A(7.481 5±1.718 0)minutes.There was significant difference between the two groups(P<0.05).No serious complication was found in both two groups postoperative.There were 4 eyes with laser damage patch at IOL,14 eyes with anterior limiting membrane rupture of vitreous body,and without influencing visual acuity.Conclusion Nd:YAG laser without mydriasis is a simple and reliable treatment for after-cataract.
