【摘要】 目的探讨Torsional超声乳化模式治疗硬核白内障的效果。方法将220例(285眼)行白内障超声乳化术的硬核白内障患者随机分为普通超声乳化模式组与Torsional超声乳化模式组,比较术中晶状体核乳化时间和灌注液用量,术后检查记录角膜内皮细胞丢失率、最佳矫正视力。结果普通模式组术中晶状体核乳化时间平均164s,Torsional模式组平均117s;灌注液用量普通模式组为93mL,Torsional模式组为41mL;2组比较差异均有统计学意义(P<0.05)。术后1周角膜内皮细胞丢失率Torsional模式组为11.8%,普通模式组为20.3%,差异有统计学意义(P<0.05)。术后第1天和第30天Torsional模式组平均最佳矫正视力为0.4和0.6,普通模式组为0.1和0.4。结论Torsional超声乳化模式治疗硬核白内障与普通超声乳化模式相比,可以减少超声乳化时间、灌注液用量,从而降低角膜内皮细胞的丢失率,提高术后视力效果。

【Abstract】 Objective To explore the effect of Torsional phacoemulsification for hard nuclear cataract.Methods A total of 220 cases(285 eyes)were randomly divided into conventional phacoemulsification mode group and Torsional phacoemulsification mode group.The ultrasound time and usage amount of perfusion fluid were compared intra-operatively;Corneal endothelial cells loss rate and best corrected visual acuity were compared between two groups post-operatively.Results The intra-operative mean ultrasound time was 164 seconds in conventional mode group,and 117 seconds in Torsional mode group;The intra-operative usage amount of perfusion fluid was 93 mL in conventional mode group,and 41 mL in Torsional mode group(P<0.05);The corneal endothelial cells loss rate was 11.8%in the Torsional mode group and 20.3% in the conventional mode group,respectively,at 1 week post-operatively(P<0.05).The post-operative best corrected visual acuity was 0.4 and 0.6 in Torsional mode group,and 0.1 and 0.4 in conventional mode group at 1 day and 30 days post-operatively,respectively.Conclusion Torsional phacoemulsification can reduce the ultrasound time,usage amount of perfusion fluid and corneal endothelial cells loss rate and improve post-operative visual acuity.
