老年肺结核继发下呼吸道革兰氏阴性菌感染的病原学及耐药性分析
ANALYSIS OF THE DISTRIBUTION AND RESISTANCE OF GRAM-NEGATIVE BACTERIA IN SENILE PEOPLE PULMONARY TUBERCULOSIS SECONDARY LOWER RESPIRATORY TRACT INFECTION
【摘要】 目的探讨老年肺结核继发下呼吸道革兰氏阴性杆菌(G-杆菌)感染的病原学和药敏情况,为临床合理选用抗生素提供参考。方法对2004~2007年间老年肺结核继发下呼吸道感染患者的痰标本细菌培养分离及药敏结果作回顾性分析。结果分离的G-杆菌中排在前3位的细菌分别为铜绿假单胞菌、鲍曼/溶血不动杆菌、肺炎克雷伯氏菌,检出率分别为30.84%、27.31%、22.47%;检出的G-杆菌大部份呈多重耐药,而总体耐药率最低为亚胺硫霉素(4.7%),其次为氧哌嗪青霉素/Tazo(9.4%),其中亚胺硫霉素对我站老年肺结核患者继发下呼吸道感染患者分离的鲍曼/溶血不动杆菌、大肠埃希菌、肺炎克雷伯氏菌、阴沟肠杆菌和铜绿假单胞菌的耐药率最低,分别为0.9%、3.4%、3.7%、5.9%、9.8%。结论老年肺结核继发下呼吸道感染的病原菌以G-杆菌为主,其对常用抗菌药物的敏感性低。
【Abstract】 ObjectiveTo discuss the pathogenic of secondary lower respiratory tract infection of gram-negative bacillus(G-bacillus) in senile people with TB gram-negative bacillus(G-bacillus) infection and drug sensitivity so as to provide a reference for the selection of antibiotics in clinic. Methods From 2004 to 2007 among hospitalized senile people TB secondary lower respiratory tract infection patients sputum culture and sensitivity results were analyzed retrospectively. Results G-bacilli in the top 3 of the bacteria were Pseudomonas aeruginosa, Baumanii / haemolytius Acinetobacter, Klebsiella pneumoniae, and the detection rate was 30.84%, 27.31%, 22.47%; All G- most were multi-drug resistant bacteria, and the lowest overall rate of resistance to IMP(4.7 %), followed by TZP(9.4%). IMP for the elderly patients in our hospital patients with tuberculosis secondary lower respiratory tract infections with the separation Bowman/hemolytic Acinetobacter, Escherichia coli, Klebsiella pneumoniae, Enterobacter cloacae and Pseudomonas aeruginosa had lower resistance. The lowest rate of drug resistance were 0.9%, 3.4%, 3.7% and 5.9%, 9.8%. Conclusion Old tuberculosis secondary lower respiratory tract infection mainly with the G-bacteria which had low sensitivity to common used anti-bacterial drug.
