回声跟踪技术评价高脂血症、高血压病、动脉粥样硬化患者颈动脉功能

【摘要】 [目的]评价高脂血症、高血压病、动脉粥样硬化患者颈动脉弹性功能变化。[方法]运用血管回声跟踪技术检测高脂血症组、高血压病组、动脉粥样硬化组和正常对照组的颈总动脉弹性参数,包括压力应变弹性系数(Ep)、僵硬系数(β)、动脉顺应性(AC)、脉搏波放大指数(AI)、脉搏波传导速度(PWVβ),并进行评价分析。[结果]和正常对照组比较,高脂血症组、高血压病组的Ep、β值、AI、PWVβ增高显著,AC显著降低(P﹤0.05)。和正常对照组比较,动脉粥样硬化组的Ep、β值、AI、PWVβ增高非常显著,AC降低非常显著(P﹤0.01)。和高脂血症组、高血压病组比较,动脉粥样硬化组的Ep、β值、AI、PWVβ增高显著,AC显著降低(P﹤0.05)。[结论]血管回声跟踪技术可以早期、准确、方便的评价高脂血症、高血压病和动脉粥样硬化患者的动脉弹性功能,高脂血症、高血压病和动脉粥样硬化患者的颈总动脉弹性功能明显降低。

【Abstract】 [Objective] To evaluate the change of carotid artery function by echotracking technique in patients with hyperlipidemia,hypertension and atherosclerosis.[Methods] A total of 1 160 cases were divided into four groups:normal control group(40 cases),hyperlipidemia group(40 cases),hypertension group(40 cases),and atherosclerosis group(40 cases).The changes of carotid artery function were evaluated by echo-tracking technique.[Results] Pressure elastic coefficient(Ep) was significantly higher in patients with hyperlipidemia,hypertension and atherosclerosis than that of normal subjects(133.61±33.89 VS 111.33±32.03,P﹤0.05;192.23±45.88 VS 111.33±32.03,P﹤0.01;222.17±75.13 VS 111.33±32.03,P ﹤0.001,respectively).Stiffness parameter(β) was significantly higher in patients with hyperlipidemia,hypertension and atherosclerosis than that of normal subjects(10.68±1.71 VS 8.21±2.19,P ﹤ 0.05;12.36±3.02 VS 8.21±2.19,P ﹤ 0.001;15.36±4.46 VS 8.21±2.19,P﹤0.001,respectively).Artery compliance(AC) was significantly lower in patients with hyper-lipidemia,hypertension and atherosclerosis than that of normal subjects(0.62±0.16 VS 0.75±0.24,P﹤0.05;0.51±0.11 VS 0.75±0.24,P﹤0.001;0.46±0.07 VS 0.75±0.24,P﹤0.001,respectively).Augmentation index(AI) was significantly higher in patients with hyperlipidemia,hypertension and atherosclerosis than that of normal subjects(17.34±9.50 VS 12.78±5.94,P﹤ 0.05;17.51±10.06 VS 12.78±5.94,P ﹤ 0.05;28.31±9.86 VS 12.78±5.94,P ﹤ 0.001,respectively).Pulse wave velocity(PWVβ) was significantly higher in patients with hyperlipidemia,hypertension and atherosclerosis than that of normal subjects(7.05±0.93 VS 6.32±0.89,P﹤0.05;8.32±0.96 VS 6.32±0.89,P﹤0.001;11.25±1.89 VS 6.32±0.89,P﹤0.001,respective-ly).The Ep,β,AI and PWVβ were significantly higher in patients with atherosclerosis than that of hyperlipidemia and hy-pertension(P﹤0.05).The AC was significantly lower in patients with atherosclerosis than that of hyperlipidemia and hyperten-sion(P﹤0.05).[Conclusion] The early change of the artery function and structure can be estimated conveniently,quickly and non-invasively by echo-tracking technique for hyperlipidemia and hypertension.Arterial elasticity function are obviously lower in patients with hyperlipidemia,hypertension and atherosclerosis.
