【摘要】 [目的]分析比较昆明市某城区和郊区1998~2003年缺血性心脏病(Ischemic heart disease,IHD)早死所致疾病负担的时间趋势。[方法]从昆明市4个区中随机抽取盘龙区和官渡区作为城区和郊区的代表,以早死所致生命损失年(years of life lost,YLL)作为IHD疾病负担的测量单位,采用3%的贴现率但不采用年龄权重分年份、地区、性别、年龄组计算标化每千人口YLL率,对其进行比较。[结果]从1998～2003年间,昆明市郊区IHD的死亡率和YLL率均呈逐年上升趋势,而城区IHD的死亡率和YLL率则随时间起伏波动;城区每年的IHD死亡率和YLL率均高于郊区。连续6年来,两个地区IHD的死亡率和YLL率均是男性高于女性;YLL率以45岁以下人群组最低,65岁及以上人群组最高且呈逐年上升趋势。[结论]应重点加强昆明市两个地区男性居民缺血性心脏病的防治工作,并采取相应措施降低老年人缺血性心脏病的死亡率。更多还原

【Abstract】 [Objective] To compare and analyze the time trend of disease burden of early death due to ischemic heart disease (IHD) in urban and suburban areas of Kunming. [Methods] Simple random sampling method was used to select Panlong district and Guandu district as study areas. Years of life lost was used to measure premature mortality burden due to IHD. YLL per 1 000 population rate and mortality rate per 100 000 population of IHD were calculated and broken down by year, area, sex and age group without age-weighting but with 3% discounting rate. These rates were standardized by age according to the direct method. [Results] From 1998 to 2003, IHD had shown a steadily increasing mortality rate and YLL rate in the sub- urban area whereas fluctuated over time in the urban area. The urban area had higher mortality rate and YLL rate of IHD than the suburban area across these six years. Predominance of males in overall YLL rates in both areas was noticed throughout the whole observed period. For both areas, the highest rates occurred among persons greater than 64 years, whereas the lowest rates occurred among persons less than 45 years of age in each of the six years. [Conclusion] In both areas of Kunming, males should be considered as a special group in further control of IHD, and measures to reduce mortality of IHD for the elderly are urgently needed.更多还原
