血清胱抑素C与β2微球蛋白、尿微量白蛋白测定在2型糖尿病早期肾损害诊断中的相关探讨
DISCUSSION OF CYSTATIN C AND Β2-MG, MALB ASSAY IN THE DIAGNOSIS OF EARLY RENAL DAMAGE IN TYPE 2 DM
【摘要】 [目的]与血β2微球蛋白(β2—MG)、尿微量白蛋白(mAlb)比较,评价血清胱抑素C(Cysctatin C)测定在2型糖尿病(DM)早期肾损害诊断中的临床应用价值。[方法]对40名健康人及82例2型糖尿病患者的Ccr、CysctatinC、β2—MG、尿mAlb测定结果进行平均值、相关性、敏感度比较分析。2型糖尿病患者按Ccr水平分为DM1组(Ccr≥80ml/min.1.73m2,34例)、DM2组(80ml/min.1.73m2﹥Ccr≥50ml/min.1.73m2,48例)。[结果]DM1组各指标平均值与对照组相比差异无统计学意义,DM2组与对照组相比则差异有统计学意义(P﹤0.01);各组结果具有良好的相关性,CysctatinC与Ccr相关系数分别为(-0.865、-0.560、-0.670)、Cysctatin C与β2—MG相关系数分别为(0.873、0.731、0.887)、Cysctatin C与尿mAlb相关系数分别为(0.803、0.686、0.842);各指标中血CysctatinC测定最为敏感(DM1组47.1%、DM2组81.2%)。[结论]血CysctatinC测定为2型糖尿病患者早期肾损害诊断的理想指标,在常规Scr、Ccr检测的同时联合应用CysctatinC检测可减低糖尿病肾病的漏诊率。
【Abstract】 [Objective] To compare β2-MG and mAlb and assess the clinical value in the detection of Cystatin C in the diagnosis of early renal damage of type 2 DM. [Methods] Detected the Ccr, Cystatin C, β2-MG and mAlb of 40 healthy per-sons and 82 cases of type 2 DM and analyzed the average value, correlation and sensitivity of the results . Cases with type 2 DM were divided into DM1 group (Ccr ≥ 80ml / min.1.73m2, 34 cases) and DM2 group (80ml / min.1.73m2 ﹥Ccr ≥ 50 ml / min. 1.73m2, 48 cases) according to their Ccr level. [Results] There was no statistical significance of the average value in DM1 comparing with the contrast group, while there was significant difference in DM2 comparing with the contrast group (P﹤0.01); the result of each group had good correlation, the correlation coefficients of Cystatin C and Ccr were -0.865, -0.560, -0.670; Cystatin C and β2-MG were 0.873, 0.731, 0.887; Cystatin C and mAlb were 0.803, 0.686, 0.842. Among the index, Cys-tatin C detection was most sensitive (47.1% in DM1, 81.2% in DM2). [Conclusion] Cystatin C detection is ideal in diagnos-ing early renal damage of type 2 DM. Combining the regular Scr, Ccr detection with Cystatin C detection, the omission diagnose rate of diabetic nephropathy can be decreased .
