慢性阻塞性肺病并发自发性气胸微创闭式引流术治疗分析
ANALYSIS OF MICRO-INVASIVE CLOSED DRAINAGE OF THORACIC CAVITY IN TREATMENT OF CHRONIC OBSTRUCTIVE PULMONARY DISEASE WITH SPONTANEOUS PNEUMOTHORAX
【摘要】 [目的]观察慢阻肺合并气胸患者行微创式胸腔闭式引流治疗疗效。[方法]慢阻肺合并气胸患者53例,随机分为治疗组与对照组,治疗组予中心静脉导管行胸腔闭式引流治疗。对照组予常规胸腔闭式引流治疗。两组均给予抗感染、氧疗等对症、支持治疗。两组患者进行治愈率、手术时间、疼痛程度、引流时间对比评定。[结果]两组在治愈率、引流时间上差异均无统计学意义(P﹥0.5)。而在手术时间、疼痛程度上差异有统计学意义(P﹤0.001)。[结论]常规胸腔闭式引流与中心静脉套管行微创式胸腔闭式引流,其2者都可有效治愈气胸,而微创式胸腔闭式引流更能减轻疼痛,减少手术时间,更易为慢阻肺患者所接受,亦更具安全性。
【Abstract】 [Objective] To observe the therapeutic effect of micro-invasive closed drainage of thoracic cavity in the treat-ment of patients with chronic obstructive pulmonary disease(COPD) combined with spontaneous pneumothorax.[Methods] 53 patients with COPD combined with spontaneous pneumothorax were randomly divided into experiment group and control group.Patients in experiment group were treated with closed drainage of thoracic cavity with central venous catheter,and pa-tients in control group accepted routine treatment of closed drainage of thoracic cavity.The two groups both accepted anti-infec-tion,oxygen therapy and other supportive treatments.The recovery rate,duration of operation,the extent of pain and the time of drainage in the two groups were evaluated and compared.[Results] There was no significance difference between the recov-ery rates and the operation duration of the two groups(P﹥0.05),but significant difference was found between the extent of pain and operation duration of the two groups(P﹤0.001).[Conclusion] The routine closed drainage of thoracic cavity and micro-invasive closed drainage of thoracic cavity had effectiveness in treatment of pneumothorax,while the micro-invasive closed drainage of thoracic cavity was more effective and safety in relieving pain and reducing the length of operation,and more acceptable by patients with COPD.
