【摘要】 [目的]探讨产后出血产妇死亡的危险因素,为产后出血的防治提供参考依据。[方法]用Logistic回归分析1988年1月～2008年6月我院368产后出血产妇的年龄、流产次数、产次、贫血、妊高症、宫缩乏力、子宫破裂、软产道损伤、胎盘早剥、前置胎盘、胎数、胎儿体重、第3产程、胎盘粘连或植入、分娩方式、手术助产、产后出血时间、产后出血量、心理因素、发病季节、文化程度等对产后出血的影响。[结果]368例产后出血产妇死亡9例,病死率为2.45%(9/368);Logistic回归分析结果显示,产后宫缩乏力、子宫破裂、软产道损伤、胎盘早剥、前置胎盘、胎盘粘连或植入、产后出血时间及产后出血量为产妇出血死亡的预防因素。[结论]宫缩乏力、胎盘粘连或植入、前置胎盘、软产道损伤、子宫破裂、产后出血时间可能是产妇出血死亡的主要危险因素。更多还原

【Abstract】 [Objective] To investigate the risk factors of mortality of puerpera with postpartum hemorrhage,so as to pro-vide reference for preventing and controlling postpartum hemorrhage.[Methods] From January 1988 to June 2008,a total of 368 puerpera with postpartum hemorrhage were recruited in the study.Logistic regression analysis were performed to analyze the effect of age,number of abortion,parity,anaemia,gestational hypertension,uterine inertia,rupture of uterus,damage of soft birth canal,placental abruption,placenta praevia,number of fetuses,birth weight,third stage of labor,placental adher-ence or implantation,delivery styles,obstetrics,time of hemorrhage,volume of hemorrhage,psychogenic factors,peak sea-sons of diseases,and education level on the postpartum hemorrhage.[Results] Out of 368 puerpera with postpartum hemor-rhage,9 puerpera died with the mortality of 2.45%(9/368);The results of logistic regression analysis showed that uterine in-ertia,rupture of uterus,damage of soft birth canal,placental abruption,placenta praevia,placental adherence or implanta-tion,time of hemorrhage and volume of hemorrhage could be the preventive factor for postpartum hemorrhage.[Conclusion] The uterine inertia,placental adherence or implantation,placenta praevia,damage of soft birth canal,rupture of uterus and time of hemorrhage could be the risk factor for the mortality of puerpera with postpartum hemorrhage.更多还原
