微螺钉种植体植入区域颌骨解剖的CBCT测量分析
Measurement of interadicular space for miniscrew insertion on cone beam computed tomography images
【摘要】 目的通过CBCT测量分析,研究下颌第二前磨牙与第一磨牙牙根间植入的微螺钉种植体与下颌管的关系及种植体周围骨质密度值,从而探讨合适的植入高度与角度。方法选择澳门仁康牙科医疗中心的38例患者的CBCT影像资料,测量下颌第二前磨牙与第一磨牙牙根间矢状面在距离参考平面10.0mm、12.0mm、14.0mm、16.0mm的4个植入高度上,以与骨皮质表面成30°、45°、60°、90°的4个方向共16个部位的颊舌侧骨质厚度,在骨厚度大于或等于6.0mm的部位模拟植入微螺钉种植体并测量其周围骨质密度。结果下颌各植入部位的颌骨厚度均数均大于6.0mm,各植入高度组中均以30°植入的骨密度值最大(P<0.05),其次是45°(P<0.05)。结论临床不同高度植入微螺钉种植体时,建议首选与骨皮质表面成30°植入、其次是45°植入,以提高微螺钉种植体的稳定性。
【Abstract】 Objective To measure the interradicular space between the mandibular second premolar and the first molar and the bone density around the circumference of miniscrews on cone beam computed tomography images(CBCT).Methods CBCT images from 38 patients were selected from Renkang Dentistry Centre,Macao.For each subject,the interradicular spaces between the mandibular second premolar and the first molar,and the buccolingual bone thickness on the median sagittal plane were measured at 4 different levels of 10.0,12.0,4.0 and 16.0mm from the reference plane and 4 angles of 30°,45°,60° and 90°.The postulating miniscrews were inserted where bone thickness was at least 6.0mm.The bone density around the miniscrew was measured.Results The mean of the bone thickness of the insertion site was more than 6.0mm.For each insertion level,the highest bone density was found when the miniscrew was inserted at 30°,followed by 45°.Conclusion Insertion of miniscrew at 30° may obtain better implant stability.
