痉挛型脑性瘫痪尖足患儿临床治疗及其踝足矫形器的应用

【摘要】 应用计算机检索Medline database(1993/2008)、中国期刊全文数据库(CNKI:1993/2008),对痉挛性脑瘫的患儿尖足的治疗方法进行总结。纳入的18篇文章显示,Bobath法、上田法、穴位按摩、肌肉松解和蜡疗、中西医结合等治疗痉挛性脑瘫的患儿尖足均取得了一定的效果,但临床可操作性差异明显。采用踝足矫形器治疗,主要是抑制异常姿势反射,促进正常姿势运动,刺激浅表的感受器和固有感受器,从而促进患儿内在因素的提高,近年来矫形器由于新的制作材料不断问世及制作水平的不断提高,已在临床上广泛应用并取得较好效果。临床应用效果显示,使用硬踝足矫形器可给患儿提供良好的髋、膝对线与对位,增加踝关节的稳定性,减轻肌张力,缓解痉挛,有效地改善步态功能。

【Abstract】 Articles about treatment methods of children with spastic cerebral palsy were summarized on Medline database and CNKI from 1993 to 2008. Among 18 articles,conventional methods including Bobath,Shangtian,point massage,muscular relaxation,and integrated traditional and western medicine have achieved for the treatment of tiptoes of children with spastic cerebral palsy to a certain degree; however,the clinical operation was poor. Ankle-foot orthosis inhibits abnormal postural reflex,promotes normal postural movement,and stimulates superficial receptor and proprioceptor so as to enhance internal agents of children patients. Recently,ankle-foot orthosis has been widely used for clinical treatment due to its development of manufacture materials and manufacture technique. Clinical application indicates that ankle-foot orthosis provides well alignment and paratope of hip and knee,enhances stability of ankle joint,relieves muscular tension and spasm,and effectively improves gait.
