不同材料铸造的义齿基托对承托区口腔黏膜微生物的影响
Effects of base denture casted with different materials on the oral mucosal microflorae of stress-bearing areas
【摘要】 目的:了解不同金属铸造基托义齿戴入前后口腔承托区黏膜表面微生物的附着变化。方法:用钛合金及钴铬合金铸造基托修复上颌肯氏I类或II类缺失患者各32例,检测患者戴牙前、戴义齿1个月、戴义齿3个月承托区黏膜面可培养菌总量、菌组成比的变化。结果:钴铬合金基托承托区口腔黏膜可培养菌总量戴牙3个月和1个月与戴牙前相比显著增多(P<0.01);钛合金基托承托区口腔黏膜可培养菌总量在各时期差异均无显著性(P>0.05);和钛合金基托相比钴铬合金基托对应承托区黏膜可培养菌总量在戴牙3个月明显增多(P<0.01)。钴铬合金基托对应承托区黏膜,乳杆菌、白色念珠菌、链球菌及放线菌组成比在戴牙1个月和3个月比戴牙前显著增高(P<0.01),而奈瑟氏菌显著下降(P<0.01);钛合金基托对应承托区黏膜,链球菌、放线菌、乳杆菌及白色念珠菌组成比戴牙3个月比备牙前显著增高(P<0.01),奈瑟氏菌显著下降(P<0.01)。与钛合金基托相比,钴铬合金基托更易促进链球菌、放线菌、乳杆菌及白色念珠菌生长,但不易促进奈瑟氏菌的生长(P<0.01)。结论:钛合金作为铸造基托材料优于钴铬合金,能减少承托区黏膜表面细菌附着,有利于口腔黏膜健康。
【Abstract】 Objective:To observe the changes in microflra atteched to the surface of oral bearing area before and after using titanium alloy base denture and co-cr alloy base denture. Methods:Sixty-four patients who were ready to accept removable partial dentures were selected and divided into two groups. Thirty-two patients used titanium alloy base denture while the other thirty-two patients used co-cr alloy base denture. Then at the following three points: before dentures were inserted,one month after wearing and three months after wearing,the total cultivable florae were counted and the proportions of bacteria detected on the denture bearing areas were calculated. Results: Co-cr alloy base denture enhanced the accumulation of microflora on stress-bearing areas three mounths after wearing(P<0.01). No did the wearing of titanium alloy base denture one mounth or three mounths after wearing(P>0.05). It was significant between the two kind base three mounths after wearing(P<0.01). The percentages of Lactobacilli, Candida albican,Actinomyces detected on stress-bearing areas also increased and Neisseria decreased significantly one mounth or three mounths after wearing Co-cr alloy base denture(P<0.01). So did the wearing of titanium alloy base denture three mounths after wearing(P<0.01). The increment of Lactobacilli and Candida albican,Actinomyces caused by Co-cr alloy base was higher than those caused by titanium alloy base denture and it was contrary to Neisseria(P<0.01). Conclusion: Titanium alloy as base material is better than Co-cr alloy. It may restrain quantity of microflora on stress-bearing areas, and is good for oral mucosal health.
