【摘要】 目的探讨老年患者经十二指肠镜逆行胰胆管造影(ERCP)的临床应用价值。方法对在我院行ERCP检查或治疗的121例次老年病人之临床资料进行回顾性分析。结果121例次ERCP检查及(或)治疗的病例中,胆总管结石79例次占65.29%,其中48例次为单发结石,31例次为多发结石;胆总管良性狭窄19例占15.70%,恶性肿瘤8例占6.61%,胆总管扩张4例占3.31%,梗阻性化脓性胆管炎3例占2.58%,支架取出3例占2.58%,先天囊肿2例占1.66%,原发性硬化性胆管炎、慢性胰腺炎和乳头病变待查各1例共占2.58%。ERCP成功110例次,成功率为90.91%,一次胆总管取石成功率79.75%,多次取石成功率91.14%,转外科取石3例。并发症发生18例次,发生率为14.88%,其中急性胰腺炎7例次,发生率为5.79%;胆管炎6例次,发生率为4.96%;高淀粉酶血症3例次,发生率为2.48%;术后乳头切口大出血1例,发生率为0.83%;肠穿孔1例次,发生率为0.83%。结论ERCP对老年胆胰疾病患者的诊疗具有良好的效果,但具有一定风险,需要积极防范并发症。更多还原

【Abstract】 Objective To study the value of endoscopic retrograde cholangiopancreatography(ERCP) in the elderly patients.Methods 121 cases elderly patients who were treated by ERCP from May 2005 to August 2008 in our hospital were analyzed retrospectively.Result In a total of 121 cases of cannulations,79 cases(occupying 65.29%) were choledocholith,including 48 cases of single common bile duct stone and 31cases of multiple common bile duct stones;19(15.70%) common bile duct of benign stricture,8(6.61%) malignant tumor;4(3.31%) choledochectasia;3(2.58%) obstructive suppurative cholangitis;3(2.58%) taken stents;2(1.66%) congenital cyst;3(2.58%) primary sclerosing cholangitis,chronic pancreatitis or nipples disease.110 cases(90.91%) of ERCP were successful with once successful rate of 79.75% and repeat successful rates of 91.14%,and 3 patients turned to surgery.Complication occurred in 18 cases with incidence rate of 14.88%,among them,7 cases(5.79%) were acute pancreatitis,6 cases(4.96%) cholangitis,3 cases(2.48%) hyperamylasemia,1 patient(0.83%) big cut bleeding,1 patient(0.83%) enterobrosis.Conclusion ERCP is an effective method in the treatment of cholecystopancreatic disease for the elderly patients,but it has some risk.So we must prevent positively the occurrence of complication.更多还原
