畸变产物耳声发射对儿童分泌性中耳炎的诊断价值

【摘要】 目的探讨畸变产物耳声发射(DPOAE)在儿童分泌性中耳炎(OME)诊断中的价值。方法对123例(234耳)OME患儿分别行DPOAE、声导抗和纯音听阈检测,比较鼓室导抗图B、C型者DPOAE各个频率的检出率及DPOAE高频检出与未检出组的气导平均阈值。结果鼓室导抗图B、C型者低、中频DPOAE的检出率差异无统计学意义(P>0.05),C型组高频的检出率明显高于B型组(P<0.05)。DPOAE高频未检出组的高、低频气导平均阈值明显高于检出组(P<0.01)。结论DPOAE高频检出率对判断儿童OME的严重程度及听力水平有重要价值。

【Abstract】 Objective To study the clinical applications of distortion product otoacoustic emission(DPOAE) to assessing children with otitis media with effusion(OME).Methods DPOAE,acoustic immitance and pure tone audiometry were administered examined in 123 children(234 ears) with OME.The DPOAE findings were compared with type B and type C tympanograms for their effectiveness of identifying the existence of OME.The average air conduction hearing levels and DPOAEs were also compared at high frequencies.Results Between the groups of type B and type C tympanograms,the difference of the DPOAE detection rates was not significant at low frequency(P>0.05),but the difference was significant at high frequency(P<0.05).The difference of the average air conduction hearing levels was significant between the positive and negative DPOAE groups at high frequency(P<0.01).Conclusion DPOAE detection rate at high frequency demonstrated clinical values to assess the severity of hearing loss in the children with OME.
