粒细胞集落刺激因子动员骨髓干细胞治疗脑梗死时间窗的研究

【摘要】 目的:研究粒细胞集落刺激因子(G-CSF)动员骨髓干细胞治疗脑梗死的最佳时间窗。方法:脑梗死造模术后24h,神经功能评分≥2分的40只大鼠随机平均分为4组。急性期给药组、亚急性期给药组,腹腔内注射G-CSF10μg·kg-·1d-1;急性期对照组、亚急性期对照组,腹腔内注射等容积的生理盐水。术后24h、10d、20d、27d行神经功能评分及体质量测定,计算体质量减轻率。术后27d计算脑梗死容积与全脑容积比,观察梗死灶病理改变。结果:术后27d体质量减轻率比较,急性期给药组小于其他3个组(P<0.05或P<0.01),亚急性期给药组小于2个对照组(P<0.05),2个对照组差异无统计学意义(P>0.05)。术后20d、27d神经功能评分比较,急性期给药组低于其他3组(P<0.05),其余3组差异无统计学意义(P>0.05)。脑梗死容积与全脑容积比值比较,急性期给药组小于其他3组(P<0.05),亚急性期给药组小于2个对照组(P<0.01)。术后27d急性期给药组可见明显的血管增生。结论:脑梗死急性期是利用粒细胞集落刺激因子动员骨髓干细胞治疗的最佳时间窗。

【Abstract】 Objective:To investigate the optimal administrative time of granulocyte colony-stimulating factor (GCSF) for mobilizing marrow stem cell to treat ischemic cerebral infarction in rats. Methods:The neurological behavioral assessments were performed in rat model of cerebral infarction after 24 hours of operation. Forty rats with scores equal to or mare than 2 were randomly divided into four groups (10 for each group). The G-CSF(10 μg·kg-1·d-1) was injected i.p. 1-10 d after operation in acute-phase group and 11-20 d after operation in subacute-phase group. The same volume of saline was injected i.p. in a-cute-phase control group and subacute-phase control group. The neurological behavioral assessments, body weight measurements and rates of body weight loss were performed on the day 1, 10, 20 and 27 subsequent to the cerebral infarction. On the 27 th day, the rats were killed and their cerebrums were taken out and stained by TTC and HE to detect the volume of infarction and pathological change. Results: The rate of body weight loss was lower in acute-phase group compared to those of other three groups (P < 0.05 or P < 0.01).There was no significant difference in rate of body weight loss between two control groups (P > 0.05). The neurological behavioral assessments of 20 d, 27 d after operation were lower in acute-phase group compared to those of other three groups(P < 0.05). And no differences were found between other three groups(P < 0.05). The ratio of the infarction volume to total cerebral volume was smaller in acute-phase group compared to those of other three groups (P < 0.05) and smaller in subacute-phase group compared to those of other two control groups (P < 0.05).There was significant pathologic change of neovascularization on the day 27 after operation in acute-phase group. Conclusion: The acute phase of cerebral infarction is the optimal administrative time of G-CSF for mobilizing marrow stem cell to treat ischemic cerebral infarction.
