【摘要】 目的探讨幽门螺杆菌(H.pylori)vacA基因亚型在海口地区胃十二指肠疾病患者中的分布。方法从176例胃十二指肠疾病患者胃黏膜标本中分离培养H.pylori,PCR法测定vacA基因亚型。结果176例患者中的基因型vacAs1 78.98%(139/176),vacAs2 15.34%(27/176),vacAm1 28.41%(50/176),vacAm2 71.59%(126/176);vacA亚型组合形式为:vacAs1/m1 28.41%(50/176),vacAs1/m2 56.25%(99/176),vacAs2/m2 15.34%(27/176),未发现va-cAs2/m1型菌株;消化性溃疡、胃癌、慢性萎缩性胃炎患者H.pylori的vacAs1、vacAm2、vacAs1/m2基因型阳性率高于慢性浅表性胃炎患者(均P<0.01),其余基因型在不同类型疾病中的分布无统计学差异(P>0.05)。结论海口地区胃十二指肠疾病患者H.pylori菌株的优势基因型为vacAs1、vacAm2、vacAs1/m2;在消化性溃疡等严重疾病患者中多见。更多还原

【Abstract】 Objective To investigate the distribution of predominant vacA subgenotypes of Helicobacter pylori(H.pylori)in gastroduodenal diseases in Haikou and study the relationship between vacA subgenotypes in H.pylori-associated diseases.Methods VacA subgenotypes of 176 H.pylori strains isolated from patients with chronic gastritis,peptic ulcer,chronic atrophic gastritis and gastric cancer were detected by polymerase chain reaction(PCR).Results The positive rate of the vacAs1,vacAs2,vacAm1,vacAm2 and vacAs1/m2 genotypes of 176 H.pylori strains was 78.98%(139/176),15.34%(27/176),28.41%(50/176),71.59%(126/176) and 56.25%(99/176)respectively.The vacAs2/m1 type was not found.The positive rate of vacAs1,vacAm2 and vacAs1/m2 genotypes was higher in patients with peptic ulcer,chronic atrophic gastritis and gastric cancer than in those with chronic gastritis(P<0.01).There was no difference in other genotypes among different gastric duodenal diseases.Conclusion VacAs1,vacAm2 and vacAs1/m2 are predominant genotypes of H.pylori strains isolated from Haikou patients,and these genotypes are dispersed in peptic ulcer,chronic atrophic gastritis and gastric cancer.更多还原
