阿尔茨海默病和血管性痴呆患者血浆同型半胱氨酸水平的差异

【摘要】 目的:探讨以血浆同型半胱氨酸(Hcy)的水平鉴别阿尔茨海默病(AD)和血管性痴呆(VD)的临床意义。方法:抽取AD患者20例(AD组)、VD患者18例(VD组)、正常体检人员20例(正常对照组)空腹静脉血各4mL,离心后使用全自动荧光偏振免疫测定法检测血浆Hcy水平,并以化学发光分析法检测血清VitB12、叶酸水平,进行组间Hcy、VitB12与叶酸水平差异比较以及三者水平相关性分析。结果:VD、AD组以及正常对照组血浆Hcy水平分别是(17.6±4.3)、(14.7±3.0)μmol/L以及(13.1±2.5)μmol/L,其中VD组与AD组、VD组与正常对照组间差异有显著性(P=0.008和P<0.001),而AD组与正常对照组间差异无显著性(P=0.129)。VD、AD组以及正常对照组间血清VitB12、叶酸水平差异均无显著性(P>0.05)。血浆Hcy与血清VitB12水平呈负相关(r=-0.340,P=0.021)。结论:VD患者平均血浆Hcy水平明显高于AD患者,提示血浆Hcy水平对于鉴别AD和VD有一定临床意义,但仍应同时结合其他因素综合考虑。

【Abstract】 Objective To investigate the differences of plasma levels of homocysteine(Hcy) in patients with Alzheimer’s disease(AD) or with vascular dementia(VD).Methods Plasma levels of Hcy in 4 mL of venous blood sample from 20 patients with AD,18 patients with VD,and 20 normal controls(NC group) were examined by flurescence polarizationimmu noasssay.Serum concentrations of VitB12 and folate were detected by chemiluminescence analysis.Comparison of Hcy,VitB12,and folate among the three groups and the correlation among Hcy,VitB12,and folate were analyzed.Results Plasma levels of Hcy were(17.6 ± 4.3) μmol /L in VD group,(14.7 ± 3.0) μmol /L in AD group,and(13.1 ± 2.5) μmol /L in NC group(VD group vs AD group,P = 0.008;VD group vs NC group,P < 0.001;AD group vs NC group,P = 0.129).Serum concentrations of VitB12 and folate were similar among the three groups(P > 0.05).Plasma level of Hcy was negatively related to serum concentration of VitB12(r =-0.340,P = 0.021).Conclusion Plasma level of Hcy is significantly different between VD patients and AD patients.
