丙氨瑞林穴位注射对子宫内膜异位症模型大鼠的镇痛作用及对P物质表达的影响
Effect on SP Expression and Analgesia of Acu-point Injection with Alarelin in Rats of Endometriosis Model
【摘要】 目的研究丙氨瑞林穴位注射对子宫内膜异位模型大鼠的镇痛作用及对P物质(SP)表达的影响。方法选取57只SD成年雌性大鼠,随机分为空白对照组(A组)、模型对照组(B组)、假手术对照组(C组)、生理盐水穴位注射组(D1组)、丙氨瑞林肌肉注射组(D2组)、丙氨瑞林穴位注射组(D3组)。建立大鼠子宫内膜异位模型,4周后用药,治疗4周后处死。分别测量用药前后痛阈的变化,免疫组化法测定SP在各组子宫及子宫内膜异位病灶标本中的表达。结果(1)B组痛阈显著低于A组和C组;D1组、D2组、D3组痛阈明显高于B组;D3组痛阈明显高于D1组和D2组,差异均有统计学意义(P<0.05或<0.01)。(2)SP主要表达于细胞浆中,呈棕黄色颗粒,分布于子宫内膜(异位和在位)上皮细胞、子宫平滑肌细胞、结缔组织、血管内皮细胞及炎性细胞中。A组、B组、C组、D1组、D2组、D3组SP表达阳性率分别为17.78%、92.22%、22.22%、57.00%、48.00%、25.00%,B组显著高于其他各组,且阳性级别最高;D3组SP表达阳性率和阳性级别低于D1组和D2组,差异均有高度统计学意义(均P<0.01)。结论小剂量丙氨瑞林穴位注射可有效抑制大鼠子宫内膜异位症引起的疼痛,其镇痛机制可能是通过抑制SP的释放,从而达到镇痛效果。丙氨瑞林穴位注射优于丙氨瑞林肌肉注射,有望成为治疗子宫内膜异位症的新方法。
【Abstract】 Objective To study the effect on SP expression and analgesia of acu-point injection with Alarelin in rats of endometriosis model. Methods Fifty-seven female ripe SD rats were divided randomly into blank group(group A),modeling group(group B),sham operation group(group C), acu-point injection with 0.9% sodium chloride injection group (group D1), in the group in which rats were performed intramuscular injection with alarelin (group D2), and in the gronp in which rats were performed acu-point injection with alarelin (group D3). Endometriosis model in rats was made, curing for 4 weeks after modeling, rats were sacrified. To measure pain threshold before and after curing, obsered the expression of substance P (SP) in uterus and endomitrium implants by immunohistochemistry method. Results (1) Pain threshold in group B was lower than that in group A and group C(P<0.01). Pain threshold in group D1, D2 and D3 was more than that in group B (P<0.05). Pain threshold in group D3 was more than that in group D1 and D2 (P<0.05). (2) SP was expressed mainly in cytoplasm, and appeared as buffy particle. SP was distributed in endometrium (ectopic and eutopic) epithelial cells, smooth muscle cells of uterus, connective tissue, vascular endothelial cell and inflammatory cells. The expression rate in group A, B, C, D1, D2 and D3 was 17.78%,92.22%,22.22%,57%,48% and 25% respectively. The expression rate and positive rank of SP in group B was the most of all (P<0.01). The expression rate and positive rank of SP in group D3 were less than those in group D1 and D2 (P<0.01). Conclusion Acu-injection with low dose of alarelin can take analgesic effect on endometriosis model rats. Suppressing the secret of SP is probably the analgesic mechanism. The curative effect of Acu-point injection with alarelin is better than intramuscular injection with alarelin, which is hoped to be a new therapy of endometriosis.
