2型糖尿病患者血清成纤维细胞生长因子21水平的变化及其临床意义

【摘要】 目的了解2型糖尿病患者的血清成纤维细胞生长因子21(FGF-21)水平,并探讨它与体脂、糖脂代谢、胰岛素敏感性及炎症因子等的相关性。方法用酶联免疫吸附法检测了35例正常对照组(NGT)、54例初诊2型糖尿病组(NDDM)和54例病程≥5年的2型糖尿病组(LDDM)患者的空腹血清FGF-21浓度,同时测定受试者的体格指标及空腹状态下血糖、血脂、胰岛素(FINS)、超敏C反应蛋白(hs-CRP)等,并计算腰臀比(WHR)、体质量指数(BMI)、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)。结果NDDM组和LDDM组的FGF-21水平均显著高于对照组,差异均有高度统计学意义(均P<0.01),但NDDM和LDDM组间的FGF-21差异无统计学意义(P>0.05)。简单相关分析显示,FGF-21与年龄、WHR、收缩压(SBP)、空腹血浆葡萄糖(FPG)、糖化血红蛋白(HbA1c)、HOMA-IR、hs-CRP均显著正相关,与HOMA-β成显著负相关。多元逐步回归分析表明,空腹血清胰岛素(FINS)、HOMA-IR、低密度脂蛋白胆固醇(LDL-C)和hs-CRP是血清FGF-21的独立相关因素。Logistic回归分析表明,FGF-21与2型糖尿病成正相关。结论FGF-21可能参与了2型糖尿病的发生和发展。

【Abstract】 Objective To investigate serum fibroblast growth factor 21 (FGF-21) levels in type 2 diabetic patients, and to explore the correlation between its serum levels and anthropometric parameters, glucose-lipid metabolism parameters, insulin sensitivity index, inflammatory factors etc. Methods Fifty-four newly diagnosed patients with type 2 diabetes mellitus (NDDM), 54 subjects with the duration of diabetes more than five years(LDDM), and 35 normal controls(NGT) participated in the study. Fasting serum FGF-21 levels were measured with an enzyme-linked immunosorbent assay. The anthropometric parameters, glucose, lipids, insulin and high sensitive C-reactive protein (hs-CRP) levels were also measured in fasting status and waist-vto-hip ratio (WHR), body mass index (BMI), HOMA-insulin resistance index (HOMA-IR) and HOMA-β-cell insulin secretion index (HOMA-β) were calculated. Results Serum FGF-21 levels were significantly higher in patients with NDDM and LDDM than in controls (P<0.01), but there was no difference in serum FGF-21 levels between NDDM and LDDM. Fasting serum FGF-21 was found to correlate positively and significantly with age, WHR, SBP, FPG, HbA1c, HOMA-IR and hsCRP, but negatively with HOMA-β. Furthermore, multiple regression analysis showed that fasting insulin, HOMA-IR, LDL cholesterol and hs-CRP were independent factors influencing serum FGF-21 levels. Logistic regression analysis demonstrated that serum FGF-21 was significantly associated with T2DM. Conclusion FGF-21 may play a role in the pathogenesis of T2DM.
