【摘要】 目的探讨颅脑损伤术中急性脑膨出的发生原因及防治措施。方法对20例术中发生急性脑膨出颅脑损伤患者的临床资料进行回顾性分析,总结其发生原因及各种防治措施的疗效。结果20例术中发生急性脑膨出患者中交通事故伤17例,重物砸伤1例,高处坠落伤2例;受伤机制:减速伤16例,加速伤3例,旋转性损伤1例;脑膨出原因:迟发性颅内血肿或血肿增大诱发9例,同侧或双侧大脑半球急性脑肿胀4例,广泛严重性脑挫裂伤4例,呼吸功能障碍、创伤性休克导致的脑组织缺血缺氧3例;伤后6月,按GOS预后分级:良好1例,中残2例,重残5例,植物生存1例,死亡11例。结论颅脑损伤术中发生急性脑膨出总体预后差,针对不同病因采取相应措施可防治颅脑损伤术中脑膨出,改善预后。更多还原

【Abstract】 Objective To discuss the cause and prophylactic,therapeutic method of acute intraoperative encephalocele in head-injured patients.Methods 20 patients with acute intraoperative encephalocele in head-injured were analyzed.Results Among the 20 cases:17 cases was traffic accident injury,1 case strike injury,2 cases crash injury;according to injury mechanism:16 cases were caused by deceleration,3 cases by acceleration,1 case by revolution;according to the reasons:9 cases were caused by delayed intracranial heamatomas,4 cases by acute diffuse brain swelling,massive and severe brain contusion and laceration in 4 cases,3 cases cerebral anoxia by respiratory dysfunction and shock;according GOS after six months:1 case recovered,2 cases were moderate disability,5 cases severe disability,1 case was in vegetative state,11 cases were dead.Conclusion The prognosis of acute intraoperative encephalocele in head-injured patients was worse.The disease should be prevented and treated effectively according to different causes.更多还原
