【摘要】 目的 探讨数字钼靶X线摄影“边缘征”对乳腺肿块样病变良、恶性的定性诊断价值。方法 74例乳腺肿块患者,均为女性,年龄19～80岁,平均49.2岁,良性36例,恶性38例,对数字钼靶X线摄影显示乳腺肿块“边缘征”进行分析。具有深分叶和/或毛刺的肿块归为恶性边缘征组,无深分叶又无毛刺的肿块归为良性边缘征组。结果 采用“边缘征”诊断乳腺恶性病变的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为89.47%、91.67%、90.54%、91.89%、89.19%。以2 cm为分界,病变大小对依靠边缘征定性良恶性病变准确性无明显差别(P>0.05)。以40岁为分界,恶性病变组:≥40岁患者89.47%,<40岁患者10.52%。≥40岁依靠良、恶性边缘征对肿瘤定性诊断准确性分别为76.92%、93.94%;<40岁,依靠良性边缘征诊断良性病变的准确性为87.50%,依靠恶性边缘征诊断恶性病变的准确性为25.00%,两者之间差别有统计学意义(P<0.05)。结论 数字钼靶X线摄影乳腺肿块“边缘征”可作为乳腺肿块样病变定性诊断的一种参考,其简单易行,具有非常大的临床价值。更多还原

【Abstract】 Objective To study the value of the "edge sign " of masses in diagnosing bearst benign and malignant lesions on digital mammography.Methods 74 women with breast lesions including benign in 36 and malignant in 38 confirmed by pathology underwent mammography,the age ranged from 19 to 80 years old,the mean age was 49.2 years old.The "edge sign " of masses on mammography was analysed,the margins of masses showed deep lobulation sign and/or spiculation were considered as malignant lesions and if no that considered as benign lesions.Results According to the "edge sign " of masses on digital mammography to diagnose the benign and malignant breast lesions,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value were89.47%,91.67%,90.54%.91.89% and 89.19%,respectively.If 2 cm in size of breast masses as a boundary,there was no difference to diagnose benign and malignant breast lesion with "edge sign"(P>0.05).Depending on the "edge sign" to diagnose breast benign and malignant lesions,89.47% and 10.52% were malignant in≥40 years old and <40 years old group respectively,the diagnosing accurate rates were 76.92% and 93.94% for benign and malignant masses respectively in over 40 years old group,while that were 87.50% and 25.00% for beingn and malignant masses in <40 years old group,respectively,there were significant differ-ences between them(P<0.05).Conclusion The "edge sign" of masses on digital mammography is of significant value in diagnosing breast masses.更多还原
