肾上腺肿瘤MR扩散加权成像分析

【摘要】 目的 探讨正常人及肾上腺肿瘤MR扩散加权成像(DWI)表现,评价DWI对肾上腺肿瘤的诊断及鉴别诊断价值。方法对10例正常人、33例肾上腺肿瘤行常规MRI及DWI检查,观察肾上腺的信号强度及测量肿瘤的表观扩散系数(ADC)值。结果(1)在DWI像上正常肾上腺表现为均匀高信号。(2)肾上腺腺瘤、嗜铬细胞瘤、转移瘤均表现为高信号,中心坏死区呈低信号,髓脂瘤表现为高低混杂信号,肾上腺囊肿表现为低信号。(3)肾上腺囊肿、腺瘤、嗜铬细胞瘤、髓脂瘤的ADC值分别为(2.896±0.114)×10-3 mm2/s、(2.009±0.08)×10-3 mm2/s、(1.903±0.076)×10-3 mm2/s、(1.861±0.073)×10-3 mm2/s。肾上腺腺瘤、嗜铬细胞瘤、髓脂瘤的ADC值差异无统计学意义(P>0.05),肾上腺囊肿与腺瘤、嗜铬细胞瘤、髓脂瘤的ADC值差异有统计学意义(P<0.05)。(4)肾上腺良恶性肿瘤的ADC值分别为(2.114±0.109)×10-3 mm2/s、(1.327±0.132)×10-3mm2/s(P<0.05)。结论DWI对肾上腺肿瘤性质鉴别有重要价值,是对常规MR检查的有益补充。

【Abstract】 Objective To study MR imaging appearances of normal adrenal gland and adrenal tumor on DWI and to evaluate the efficacy of DWI in diagnosis and differentiating diagnosis of adrenal tumor.Methods 10 cases of normal subjects and 33 cases of adrenal tumors underwent conventional MRI and DWI examinations.The signal intensity of adrenal glands were observed and apparent diffusion coefficient(ADC) values of tumor were measured.Results(1) The normal adrenal glands were hyperintense on DWI;(2)Adrenal adenomas,pheochromocytoma and metastatic tumors showed marked high signal intensity,the necrotic or cystic parts of tumors demonstrated low signal intensity,adrenal myelolipoma showed mixed signal intensity,adrenal cyst showed low signal intensity;(3)The mean ADC values of adrenal cyst,adenomas,pheochromocytomas and myelolipomas were(2.896±0.114) ± 10-3 mm2/s,(2.009±0.08) × 10-3mm2/s,(1.903±0.076) × 10-3 mm2/s and(1.861±0.073) ×10-3 mm2/s,respectively.There was no significant difference of ADC values between adrenal adenomas,pheochromocytomas and myelolipomas(P>0.05).There was a significant difference of ADC values between adrenal cyst and adenomas,pheochromocytoma and myelolipoma(P<0.05);(4) The mean ADC values of benign adrenal lesions and malignant ones were(2.114±0.109) × 10-3 mm2/s and(1.327±0.132) × 10-3 mm2/s(P<0.05).Conclusion DWI is valuable in differe,ntiating adrenal tumors,and is a useful supplement for conventional MRI examination
