螺旋CT对胸腺上皮肿瘤侵袭性的诊断价值探讨

【摘要】 目的 探讨螺旋CT对胸腺上皮肿瘤侵袭性的诊断价值。方法 回顾性分析77例手术病理证实为胸腺上皮肿瘤的术前CT资料,对病灶的边缘,胸膜情况,心包以及血管情况等进行观察记录,并与手术记录、病理进行对照。结果 包膜完整者较无包膜者更多地呈光滑边缘(P<0.01),包膜侵犯组与包膜未侵犯组边缘情况无统计学差异。CT对胸膜侵犯、心包侵犯、血管侵犯、肺侵犯的敏感性、特异性、准确性分别为:76.6%、78.7%、77.9%;52.6%、84.5%、76.6%;100%、82.1%、84.4%;57.1%、96.8%、89.6%。结论 CT对于胸膜、心包、血管和肺侵犯的判断比较准确,对于心包和肺受侵的敏感性不足。

【Abstract】 Objective To explore the diagnostic value of spiral CT in identifying the invasion of thymic epithelial tumors.Methods Seventy-seven patients with thymic epithelial tumors confirmed by surgery and pathology were retrospectively analyzed.All of them underwent computed tomography before surgery.CT findings including mass margins and invasion of pleural,pericardium,and vessels were compared with surgical records and pathologic examination.Results Encapsulated tumors were more likely to have smooth margins than that with no encapsulation(P<0.01).There was no statistically difference between the tumors with encapsulation invasion and no encapsulation invasion.The sensitivity,specificity,and accuracy of CT in identifying pleural invasion,pericardium invasion,vessels invasion,and lung invasion were 76.6%,78.7% and 77.9%;52.6%,84.5% and 76.6%;100%,82.1 % and84.4%;57.1 %,96.8% and 89.6% respectively.Conclusion CT in evaluating the invasion of pleural,pericardium,vessels,and lung by thymic epithelial tumors is credible,but the sensitivity in identifying the invasion of pericardium and lung is not well.
