创伤后应激障碍早期灰质体积及DTI改变

【摘要】 目的 探讨早期创伤后应激障碍(PTSD)患者边缘系统灰质体积及磁共振扩散张量成像(DTI)的改变。方法 矿难发生6个月后,对10例PTSD患者、10例年龄、性别和应激暴露均与PTSD组相同但未产生PTSD者作对照进行MRI扫描,获取脑结构的高分辨T1加权像及DTI像,分别进行测量分析,比较2组边缘系统的灰质容积、部分各向异性指数(FA)值及表观弥散系数(ADC)值的异同。结果 在考虑年龄和性别的影响后,PTSD组与对照组的FA值在右侧后扣带回的差异有统计学意义(P<(0.05);2组边缘系统的灰质体积、ADC值及其它区域FA值差异无统计学意义。结论 早期PTSD患者右侧后扣带回FA值减小,提示上述脑区存在结构改变,DTI可作为反映这些神经网络病理性改变的较敏感手段。

【Abstract】 Objective To investigate the changes of the volume of gray matter in the limbic system and diffusion tensor imaging(DTI) in patients with early post-traumatic stress disorder(PTSD).Methods 10 patients with PTSD caused by mining disaster were involved 6 months later in this study,and 10 healthy persons with the same sex/age and exposed to the same mining disaster were selected as controls.The high resolution T1 weighted images and DTI data were gained by using MR imaging scan.Then the volume of gray matter in the limbic systems,fractional anisotropy(FA) and apparent diffusion coefficient(ADC) values of the limbic system were measured and the differences between this two groups were analysed.Results In comparison with controls,the PTSD patients had diminished FA values in right posterior cingulate(P<0.05).FA values in other regions and the volume of gray matter and ADC values in the limbic system did not show any significant differences between two groups.Conclusion The results suggest that there may be morphometric changes in right posterior cingulate in patients with early PTSD.The changes of FA values detected by DTI can help to determine the involvement of neural network in PTSD patients sensitively.
