鼻咽癌患者放疗后耳聋的危险因素和临床处理

【摘要】 目的:了解鼻咽癌患者放疗后耳聋的影响因素探索耳聋的处理方法。方法:观察220例鼻咽癌患者放疗后耳聋的发生情况,对耳聋者鼓室行穿刺抽液。分析患者性别、年龄、鼻咽肿瘤T分期、侧壁侵犯、放疗前分泌性中耳炎等因素与放疗后出现耳聋的关系。结果:鼻咽癌患者放疗后耳聋的发生率为46.82%,放疗后6个月内发生率为52.42%。患者性别、年龄、鼻咽肿瘤T分期等因素对放疗后耳聋的发生无显著性影响(P>0.05),肿瘤侵犯鼻咽侧壁和放疗前有分泌性中耳炎与放疗后耳聋的发生显著相关(P<0.01)。结论:肿瘤侵犯鼻咽侧壁和放疗前有分泌性中耳炎是放疗后耳聋的危险因素。鼻咽癌患者放疗后耳聋的处理目前比较好的是重复鼓室穿刺抽液配合适时倒佩戴助听器。

【Abstract】 Objective:To analyze the deafness in nasopharyngeal carcinoma (NPC) patients after radiotherapy and to explore the appropriate treatment.Methods: Observe the incidence of deafness in 220 NPC patients after radiotherapy.Puncture the tympanic membranes of deaf ears and draw out the fluid.Analyze the relationship between sex,age,T stages of nasopharyngeal tumors,invasions of side walls,secretory otitis media before radiotherapy and deafness after radiotherapy. Results: The incidence rate of deafness in NPC patients after radiotherapy was 46.82% and 52.42% 6 months later.Sex,age,T stages had no significant influence on deafness after radiotherapy (P>0.05).The invasions of side walls and secretory otitis media before radiotherapy had significant relationships with deafness (P<0.01).Conclusion: The incidence rate of deafness in NPC patients after radiotherapy is high.Sex,age,T stages have no relationship to deafness.The invasions of side walls and secretory otitis media before radiotherapy are the risk factors of deafness after radiotherapy.The appropriate treatments of deafness in NPC patients after radiotherapy are puncturing the tympanic membranes and drawing out the fluid repeatedly cooperating with hearing aids on proper time.Accurate radiotherapy technology including intensity modulated radiation therapy (IMRT) and imaging guided radiation therapy (IGRT) may decrease the incidence of deafness of NPC patients.
