5-FU经不同途径区域化疗影响大鼠肝癌血管生成的研究

【摘要】 目的:通过对大鼠移植性肝癌进行不同途径区域灌注化疗,研究不同给药途径下化疗对肿瘤血管生成的影响。方法:建立大鼠移植性原位肝癌模型,分别经大鼠外周静脉,肝动脉,门静脉,肝动脉结扎的门静脉注射5-FU,给药剂量为20mg/kg,连续给药4天,采用免疫组化法检测VEGF和CD34在不同途径区域化疗下肝癌组织中的表达及光镜下计算肿瘤内微血管密度。结果:外周静脉组全身化疗的肝癌VEGF表达显著高于其它3个区域性灌注化疗组,区域性灌注化疗的3个化疗组中,门静脉组的VEGF表达明显高于肝动脉组和结扎肝动脉的门静脉组(P<0.05);结扎肝动脉的门静脉组肝癌组织的CD34蛋白表达和MVD最低,明显低于其他四组(P<0.05)。结论:区域性灌注化疗可有效抑制肝癌血管生成;区域性灌注化疗抗肿瘤血管生成的机制可能是抑制肿瘤组织VEGF的表达,减少肿瘤组织内血管生成,并使MVD降低,从而抑制肿瘤生长。

【Abstract】 Objective:To study the effects on the angio-genesis in hepatocellular carcinoma through peripheral venous in-jection and various routes of regional perfusion of chemotherapy.Methods:Establishment of the rat hepatocellular carcinoma model.Groups were infused 5-FU through peripheral vein(jugular vein),hepatic artery and portal vein,respectively.The 5-FU dose of rats was 20mg/kg for 4 days successively through the corresponding routes.Detection of the expression of VEGF protein and CD34 pro-tein in HCC and count the number of microvessel density in the microscope.Results:It showed that the VEGF expression of the pe-ripheral vein group was higher than the other three chemotherapy groups,and the difference is statistically significant.Among the three regional infusion chemotherapy groups,the VEGF expression of portal vein group is significantly higher than the group of portal vein with hepatic artery ligation and the hepatic artery group(P< 0.05).The CD34protein expression and MVD of the group of portal vein with ligated hepatic artery in HCC were the lowest,and the difference was significant compared with the other four groups(P <0.05).Conclusions:Regional infusion of chemotherapy could inhibit the angiogenesis of HCC effectively.The mechanisms of the regional chemotherapy in inhibiting the angiogenesis of HCC may be through inhibiting the VEGF expression and reducing the angiogenesis to make MVD reduce,so as to inhibit the growth of HCC.
