【摘要】 目的探讨依赖利福平结核分枝杆菌(依R菌)Rv0341天然蛋白检测血清Rv0341抗体,诊断依R菌感染肺结核病的价值。方法典型依R菌经SDS-PAGE、转膜后,获得Rv0341天然蛋白带,采用结核分枝杆菌抗体DIGFA诊断试剂检测血清Rv0341抗体,并考察该方法的重复性、特异性及敏感性。结果利用Rv0341天然蛋白检测血清Rv0341抗体,具有良好的批内和批间重复性;血清Rv0341抗体在耐R菌肺结核组和R敏感菌肺结核组的特异性分别为92.3%(24/26)和79.3%(23/29),在依R菌肺结核R治疗组中的敏感性为88.9%(16/18);在培养阴性结核组中,阳性率为23.8%(29/122)。54份阳性结果血清进行重复实验,确认为阳性。结论Rv0341天然蛋白可作为诊断抗原用于依R菌结核病血清学诊断试剂盒的研发,以快速辅助诊断依R菌结核病,特别对菌阴依R菌结核病更有诊断价值。更多还原

【Abstract】 Objective To investigate the significance of determination of serum Rv0341 antibody with natural Rv0341 protein of rifampin-dependent M. tuberculosis in diagnosis of tuberculosis. Methods The Rv0341 protein of typical rifampin-dependent M. tuberculosis was identified by SDS-PAGE, then transferred to NC membrane and used as an antigen for determination of serum Rv0341 antibody by DIGFA diagnostic kit. The developed method was evaluated for reproducibility, specificity and sensitivity. Results The developed method for determination of serum Rv0341 antibody with natural Rv0341 protein showed good inter-and intrareproducibility. The specificities of the method for determination of serum Rv0341 antibody in rifampin-resistant and rifampin-sensitive M. tuberculosis were 92. 3%(24 / 26)and 79. 3%(23 / 29)respectively. The sensitivity of the method for determination of serum samples of rifampin-dependent M. tuberculosis after treatment with rifampin was 88.9%(16 / 18). Of the 122 samples negative for rifampin-dependent M. tuberculosis by bacterial culture, 29 (23.8%)were positive by the developed method. The 54 positive samples screened by the method were further confirmed as positive by repeat test. Conclusion Natural Rv0341 protein may be used as an antigen for development of serological diagnostic kit for tuberculosis caused by rifampin-dependent M. tuberculosis. The determination of Rv0341 antibody with the protein may be used as an assistant diagnostic method for rapid screening of rifampin-dependent M. tuberculosis, especially in the samples negative in bacterial culture.更多还原
