上海市虹口区15岁以上人群麻疹抗体水平监测与分析

【摘要】 目的了解上海市虹口区15岁以上人群麻疹抗体水平和麻疹发病情况,为制定成人麻疹免疫策略提供依据。方法定量ELISA法检测本区及外来15岁以上人群的麻疹抗体水平,与微量细胞中和法检测结果进行比较,并对1998~2007年本区和外来16岁以上人群麻疹发病资料进行分析。结果本区和外来15岁以上人群麻疹抗体平均阳性率分别为96.83%和98.21%,麻疹抗体GMT平均水平分别为1217.86和1553.73IU/ml,本区各年龄组抗体GMT水平15~19岁组最低,40岁以上组最高;外来各年龄组间抗体GMT水平差异无统计学意义,15~19岁组和30~39岁组麻疹抗体水平显著高于本区人群;ELISA法与微量细胞中和法检测结果呈正相关,回归方程有统计学意义,15岁以上人群中仅有83.5%达到麻疹抗体保护性水平;近10年来,本区与外来16岁以上人群中麻疹发病比例有逐年增高的趋势,本区发病比例显著高于外来人群。结论为消除麻疹,有必要制定成人麻疹免疫策略,其中本区成人麻疹防制策略较外来成人更为紧迫。

【Abstract】 Objective To investigate the measles antibody level and onset of measles in the populations aged more than 15 years in Hongkou District,Shanghai City,China and provide a basis for immune strategy of measles in adults.Methods The measles antibody levels in native residents and the populations from another places,aged more than 15 years,in Hongkou District,Shanghai City were determined by quantitative ELISA,and the results were compared with those by micro-neutralization test.Meanwhile,the data on onset of measles in the populations aged more than 16 years in the said district and from another places during 1998 ~ 2007 were analyzed.Results The measles antibody positive rates in native residents and the populations from another places were 96.83% and 98.21%,while the antibody GMTs were 1 217.86 and 1 553.73 IU /ml,respectively.The lowest antibody GMTs of native residents were observed in the age group of 15 ~ 19 years,while the highest ones in age group of more than 40 years.However,the anti-body GMTs in the populations from another places showed no significant difference in various age groups.The antibody GMTs of populations from anthor places in the age groups of 15 ~ 19 years and 30 ~ 39 years were significantly higher than those of native residents.The determination results by ELISA were positively related to those by micro-neutralization test,and the regression equations showed a significant difference.Protective antibody levels were observed in only 83.5% of populations aged more than 15 years.The morbidity of measles in the district increased year by year in the past 10 years,and was significantly higher in native residents than in the populations from another places.Conclusion In order to eliminate measles,it is necessary to develop a immune strategy of adults in the district,especially in the native residents.
