颅内后交通动脉宽颈动脉瘤支架辅助弹簧圈栓塞治疗
Combined implantation of intracranial Neuroform stent and endovascular coil in treatment of wide-necked posterior communicating artery aneurysms
【摘要】 目的探讨应用自膨胀颅内专用支架(Neuroform支架)与弹簧圈栓塞相结合治疗后交通动脉宽颈动脉瘤技术的临床应用价值。方法35例患者(35个颅内后交通动脉宽颈动脉瘤),其中男性16例,女性19例,年龄32~70岁,平均年龄51岁。采用Neuroform支架与电解可脱微弹簧圈(GDC)结合栓塞治疗。通过微导管释放Neuroform支架覆盖动脉瘤瘤颈,将另一微导管通过支架网孔进入动脉瘤腔以GDC栓塞动脉瘤。结果全部病例采用Neuroform支架结合GDC栓塞,其中28例致密栓塞,7例部分栓塞。仅2例术中发生支架内血栓,其余患者均恢复良好,出院。24例术后3个月血管造影随访:其中19例致密栓塞的动脉瘤均未显影,载瘤动脉通畅;5例动脉瘤颈有残留。结论Neuroform颅内支架使用安全,适用于颅内后交通动脉宽颈动脉瘤的支架辅助弹簧圈栓塞,特别适用于迂曲的脑血管。
【Abstract】 Objective To summarize the technique and the clinical value of Neuroform stents combined with Guglielmi detachable coil(GDC) in treatment wide-necked posterior communicating artery aneurysms. Methods Thirty-five wide-necked posterior communicating artery aneurysms were performed, male 16, female 19, aged 32-70 years, mean age 51 years, 35 endovascular procedures by intracranial stent and GDC. The Neuroform stents were deployed across the neck of aneurysms. Another micro catheter was introduced into the aneurysm sac in advance or through stent mesh and then detachable coils were released to embolize the aneurysms. Results All patients were identified as having wide-necked intracranial aneurysms suitable for stent-as- sisted GDC treatment. Total occlusion in 28 patients and partial occlusion in 7 patients were achieved. Instent thrombosis occurred in 2 patients. The others recovered well. Twenty-four patients were angiographically followed-up for 3 months after the procedure. Among them,no filling were shown for the 19 aneurysms which were densely packed and 5 aneurysms with neck remnant. Conclusion The Neuroform stent is a very easy and safe intracranial stent-assisted coil for embolization the wide-necked intracranial aneurysms. It is very suitable for treating the wide-necked posterior communicating artery aneurysms with severely tortuous intracranial artery.
