扩散加权成像在小儿病毒性脑炎早期诊断中的应用
The application of DWI in diagnosis of early viral encephalitis in children
【摘要】 目的探讨扩散加权成像(DWI)在早期小儿病毒性脑炎诊断中的影像学特点及其临床意义。方法分析23例经临床确诊的1~14岁病毒性脑炎患者脑部DWI及常规MRI表现特点,根据常规MRI与DWI病变异常信号的范围和数目差别将患者分为两组,比较病变区与正常脑组织及组间ADC值差别,判断ADC值变化与发病时间(TI)之间的相关关系。结果常规MRI检查在发病早期无论从病灶的数目、范围及信号强度均不如DWI图像显示清楚,组1患者ADC值明显低于组2及正常脑组织ADC值(P<0.05),病变区域ADC值降低程度与发病时间存在相关性(r=0.827,P<0.05)。结论DWI可早期发现病毒性脑炎病灶,对病毒性脑炎的定位、定量判断病情严重性及预后评价具有重要价值。
【Abstract】 Objective To study DWI presentative characteristics and its clinical diagnostic value of early viral encephalitis in children. Methods MRI manifestations of 23 patients (ranged from 1 to fourteen years old) with viral encephalitis confirmed clinically were retrospectively analyzed. Based on the qualitative and quantitative comparison of the conventional MRI and DWI,the patients were divided into two groups. Apparent diffusion coefficient (ADC) values of the involved and normal brain tissues were compared for each group. The degree of correlation between the time (TI) from the onset of neurological symptoms to the MR examination and ADC values was determined. Results During the early time from the onset of neurological symptoms,DWI was superior to conventional MRI in detecting the encephalitic involved sites and in depicting the borders of the encephalitic lesions.ADC values of affected cerebral tissue were significantly lower than those in group II and normal brain tissues (P<0.05). There was an excellent correlation between ADC values and duration of the disease (r=0.827,P<0.05). Conclusions DWI is superior to conventional MR sequences in the detection of early viral encephalitic lesions,DWI has high value in the location of focus of infection,evaluation of severity,and the prediction of prognosis in children with viral encephalitis.
